ASAP Veterans Summit: June 14 th, 2016 Veteran Master Leasing Program Pro Forma Samaritan will enter into a modified Triple Net Lease (Master Lease Agreement) with the building owner for an amount equal to $1500 each per month for 1 2BR Apartment plus $1800 each per month for 10 3BR Apartments. As calculated, the total annual rent will be $234,000 or $19,500 per month. Under the terms of the agreement, Samaritan will be responsible for payment of Real Estate Taxes, Heating Fuel, Utilities, Water/Sewer Charges, and Liability Insurance. Samaritan will also be responsible for interior repairs in rooms and common areas, as well as cleaning of common areas, trash removal and snow removal. Under the terms of the agreement, building owner will be responsible for payment of Fire and Building Coverage Insurance. Building owner will also be responsible for exterior building repairs, repairs to building systems (Boilers, Hot Water Heaters, Fire Alarm, & Sprinkler System) and all required periodic systems inspections. Term of lease will be five years with an additional five year renewal option. Rent escalation as mutually negotiated and agreed to by the parties will be included for the renewal option term. Samaritan will provide on-site Half Time Case Management Services. Samaritan will coordinate Property Management & Maintenance Services. Samaritan will provide On Site Security through a security services contract. One security officer will be on site from 8 AM to 10 PM seven days a week. Samaritan will provide appropriate tenant furniture for each tenant apartment. Samaritan will receive a Management Fee equivalent to 15% of Rent Revenue. A total annual operating subsidy equivalent of $308,234 is required under the terms and conditions as set forth above

8. ASAP Veterans Summit: June 14 th, 2016 Success Story Donna was one of the first to walk through the doors of the Samaritan Village Women Veterans Program. Despite growing up with many luxuries in a beautiful, stable home with strong parental involvement, Donna began experimenting with drugs and alcohol in her late teens. After five years of service in the Army, her transition to civilian life included substance abuse to ease the stress and struggles of her circumstances, which included loss of her marriage, family and job. Samaritan Village provided her the safe environment and stability to face her addiction and experiences.
