Objective 10: Considering the potential for housing support, housing-related services and telecare to support people with dementia and their carers. The needs of people with dementia and their carers should be included in the development of housing options, assistive technology and telecare. As evidence emerges, commissioners should consider the provision of options to prolong independent living and delay reliance on more intensive services

7 Priority Objectives Objective 2: Good-quality early diagnosis and intervention Objective 6: Improved community personal support services Objective 7: Implementing the Carers’ Strategy Objective 8: Improved quality of care for people with dementia in general hospitals Objective 11: Living well with dementia in care homes Objective 13: An informed and effective workforce Objective 14: A joint commissioning strategy for dementia Reducing the use of Antipsychotics

Challenge….to keep ‘Housing’ on the Dementia map

Housing – a true partner in Reablement ISSUES: - Artificial silos between social care, health, housing, primary care Narrow minded approach that only focuses on ‘risk’ Perception that increasing independence may place the person at risk Extra care housing appropriate for people with dementia Consider how dementia expertise can be integrated more fully and effectively into all elements of a reablement services (that includes assistive technology and housing) OPPORTUNITIES:- Opening the dialogue, widening the debate, sharing the financial challenge More imaginative use of resources linked to managing risk Using technology to its full potential in order to manage risk Use of short term tenancies to offer reablement opportunities. Housing as part of community ‘assets’ that can support people with dementia to ‘live well in their communities’

Housing – a true partner in Reablement ISSUES: -  Review and improve communication and awareness with other organisations to achieve integrated working for better outcomes  Engaging and educating GPs and Consortia on the value of reablement and the role housing and technology have to play OPPORTUNITIES:-  Further work with ALL agencies who are willing and able to work to a enablement model to deliver as part of reablement
