5 NHS Norfolk NHS Great Yarmouth and Waveney What the consultation told us Widespread support for the general priorities A greater priority for improved community support services for people living at home (including intermediate care) More detail on how progress will be monitored and reported More emphasis on how joint commissioning will work in practice More focus on the timing of improvements within the pathway The need for a personalised service More clarity about the financial profile

8 NHS Norfolk NHS Great Yarmouth and Waveney What we have done so far Professional & public awareness improvements ‘Memory treatment' services operating in each locality Primary care link worker for older people pilot Dementia Advisor pilot across all Norfolk localities Draft Carers strategy consistent with joint Norfolk dementia strategy Each Acute General Hospital has a nominated lead clinician for dementia

9 NHS Norfolk NHS Great Yarmouth and Waveney And….. ‘This is me’ booklets are being used Mental health liaison nurses in acute settings Protected meal times on all wards Workshops for Care Home Managers across all sectors Reduction in the use of anti-psychotic drugs in Care Homes NWMH NHS FT established in-reach support to Care Homes SWIFT workforce development plan project for MH including dementia

Increase Memory Clinic Capacity Improve /Increase Educational Support & Materials Improve Capacity in Cognitive Stimulation Therapy Improve Prescribing Raise GP Awareness Improve services for people with dementia in Acute Hospitals Support initiatives that will improve the care people receive in Care Homes Review Community & Day Care including where appropriate respite
