4 WHO CAN APPLY for HACC? Frail aged people People with a disability Carers of frail aged or person with a disability And you live at home

5 WHO CAN APPLY for CACP? The person is eligible to receive community care only if the person: is assessed by ACAT ( eligible for care equivalent to low level residential care; and prefers to remain living at home; and is able to remain living at home with the support of community care.

6 WHO CAN APPLY for EACH? Extended Aged Care in the Home Needs a high level of care Extended Aged Care in the Home (Dementia) Needs a high level of care and has dementia
