3 Fresno City College  Registered Nursing Department Requires a Completed Packet:  Have you ever attended the FCC RN, LVN or LVN to RN Articulation Nursing Program? __Yes __No If yes, give dates of attendance:___________________  Do you have a Bachelor’s or higher level degree? ____Yes ____No  Are you a Veteran? __Yes __No Spouse of Veteran? __Yes __ No (DD214 form must be submitted)  Have you completed the ATI TEAS V version exam with a 62% or higher? ____Yes ____No. (If yes, please include a copy of your passing test scores with your application AND transfer your scores to Fresno City College through the www.atitesting.com website. If no, application requirements for the program are not met.) TEAS Testing – Students applying to the Traditional LVN to RN Articulation Nursing Program are required to complete the Test of Essential Academic Skills (TEAS – Version V) exam prior to application. This test was developed to measure basic essential skills in the academic content area domains of Reading, Mathematics, Science, and English and Language Usage. For any additional information on the TEAS Exam, please visit www.atitesting.com. PLEASE NOTE: All students applying to the Registered Nursing program are required to submit a copy of their TEAS exam results with their Nursing application Under special circumstances, applicants who have successfully completed the TEAS exam with a passing score of 62% or higher may be considered to enter the RN program if any new openings become available due to federal grant funding.www.atitesting.com  Completed ASF form. Please list all colleges attended on the ASF including Fresno City College and all affiliated campuses. Please note: Failure to list all colleges previously attended may result in your application being denied.  Student ID Number. All applicants to the Nursing programs must apply for admission to the college to obtain a student ID number prior to applying to the program. The seven-digit ID number must be noted on the Academic Summary Form (ASF) at the time of application. Please refer to the attached document titled “Application Process for Admission” for assistance on this process.

4 Fresno City College Registered Nursing Department Requires a Completed Packet:  Official College Transcripts are necessary for a complete packet.  Two (2) official, up to date, sealed transcripts from all colleges attended (outside of the State Center Community College District).  One (1) official, current transcript from State Center Community College District (including Fresno City College, Reedley College, Madera Center, Clovis Center, Oakhurst Center, Kerman Center, Willow- International Center).  Please request that the transcripts be mailed to you first, then forward the packet as a whole to the Division office. DO NOT SEND TRANSCRIPTS SEPARATELY. TRANSCRIPTS MUST ACCOMPANY THE ASF FORM. (Note: When requesting transcripts, please obtain a copy for your records so you will be aware of the content.) Please note that it may take 4-6 weeks to obtain transcripts, so do not wait until the last minute.  An unofficial copy of your high school transcripts; or diploma; or GED certificate.

5 Fresno City College  Registered Nursing Department’s LVN to RN Articulation PrograM  Fresno City College offers two options for Licensed Vocational Nurses (LVN) articulating to become Registered Nurses (RN). 1.Associate Degree in Nursing 2.30 Unit Option   The program for California Licensed Vocational Nurses requires one academic year (Three semesters) of the Registered Nursing Program. LVN to RN students enter the third semester of the RN curriculum but take a different sequence of courses from traditional RN students.

6 Fresno City College  If an LVN wishes to obtain an Associate of Science Degree and be considered a graduate of Fresno City College Registered Nursing Program they must successfully complete all nursing requirements, including the prerequisites and general education requirements listed below with a grade of “C” or better. A cumulative GPA of 2.5 or above is required for admission to the program. California Licensed Vocational Nursing license required.  LVN Associate Degree students are subject to the same educational standards and lottery selection process education standards as the basic RN students.  LVN/RN AS Degree Articulation Plan Prereqisite Requirements Units Biol. 20 - Human Anatomy 4 Biol. 22 - Human Physiology 5 Math 103 - Intermediate Algebra or a higher math course 5 Chem. 3A - Introduction to General Chemistry or equivalent4 Biol. 31 - Microbiology5 English 1A 4 Psychology 2 - General Psychology3 Sociology 1A - Introduction to Sociology3  General Education Requirements Units Comm. 1 or 2 or 83 2 PE courses2 Political Science 2 or 1103 Humanities Elective3 Associate Degree in Nursing

7 Fresno City College  According to the BRN interpretation of CCR 1429 an LVN who wishes to become an RN using the 30 Unit Option is required to complete no more than 30 semester units in nursing and related courses listed below with a grade of “C” or better. Twenty of the units are nursing courses and ten units are in physiology and microbiology.  The 30-unit option is unique to California and some states will not issue a license to an individual who was originally licensed in California under the 30 unit Option regulations. Students completing the 30 unit option are eligible to take the NCLEX-RN exam but do so as non-degree candidates. Students selecting the “30 Unit Option” are not considered graduates of Fresno City College as per the requirements of Fresno City College and the Board of Registered Nursing CCR 1429. Once the student has completed the 30 unit option and received an RN license from the BRN, the BRN cannot change the status to that of a graduate even if the student goes on to acquire a degree in nursing at a later date.  30 unit option students are subject to the same educational standards as the basic RN students.  California Licensed Vocational Nursing license required.  GPA must be at 2.0 or higher  The 30-unit option LVN tract is only available on a space available basis.  A certificate is awarded at the end of the LVN to RN Articulation Plan LVN/RN “30 Unit Option” Prerequisite RequirementsUnits  Biol 22 - Human Physiology 5  Biol 31 - Microbiology5 “30 Unit Option” Non-degree LVN-RN Articulation Plan:

9  A minimum of twelve (12) units in residence at district colleges or centers, with attendance during the last semester prior to graduation, or  A total of forty-five (45) units in residence at district colleges or centers if not in attendance during the last semester prior to graduation. 10 Fresno City College  It is recommended that the LVN student take a LVN Transition course, the Transcultural Nursing RN 33, as well as a refresher course, Specific special studies courses in Maternity and Pediatric Nursing using RN 205 Care of the Childbearing and Parenting Family Theory and RN 206 Care of the Childbearing with Parenting Family Simulation should be taken prior to taking the NCLEX-RN licensing examination, as the content is included in the testing for RN licensure but not in this articulation plan  Please see the Director of Nursing – Stephanie R.
