2 CHAPERONES, TEAM CAPTAINS, AND BUSES Chaperones will be provided training, assigned specific teams of students, and given detailed daily procedures/ agenda prior to the trip. Roommates have been selected and put into teams. Each team will be assigned to a chaperone and bus. Students WILL NOT have their choice of bus. Students will be issued ID badges with emergency contact information prior to leaving. Team captains have been selected by administration and staff for each team. −Captains will be responsible for holding the room key, taking team attendance on the bus, in the room, and at various sites. Captain’s will immediately report concerns to the team’s chaperone.

3 DEPARTURE Departure will be promptly at 6:30 AM on Wednesday, June 1 st. Students are to arrive no later than 6:00 AM. Students are to go directly to their assigned area and check in with their chaperones. −Blue Team will meet in the gym. −White Team will meet on the 8 th grade side of the cafeteria (closest to the windows). −Red Team will meet on the 7 th grade side of the cafeteria (closest to the gym). Students are to REMAIN IN THEIR ASSSIGNED AREA until dismissed to load the busses.

7 RETURN Friday, June 3 rd about 9:30 PM Parents are EXPECTED to be at LBMS no later than 9:30 PM to pick up their child(ren) −We will send bus updates via Twitter and have students call informing parents of the time of arrival. Avoid parking in the bus circle. Designate a place to meet your child upon return.

8 BEHAVIOR WE EXPECT AND WILL ACCEPT ONLY EXCELLENT BEHAVIOR FROM ALL STUDENTS THROUGHOUT THE ENTIRE TRIP. Representatives of both Lee Burneson Middle School and Westlake. Parents are asked to emphasize the importance of responsible and courteous behavior with students. Students must realize decisions are made and instructions are given with the safety and concern of the entire group in mind. Any student who fails to adhere to our code of conduct may face a discipline hearing with the possibility of being sent home at the expense of his/ her family.

9 PERSONAL BELONGINGS Students are solely responsible for ALL of their belongings. Lost articles, including wallets, purses, and money ARE NOT the responsibility of the tour company, chaperones, and /or Westlake City Schools.

10 LUGGAGE Each student is permitted one normal size suitcase (19”x26”) and one “soft” carry-on bag. −Luggage should have identification tags and be clearly marked. −Students will be expected to carry their own luggage to the bus and to their rooms at the hotel. Parents are required to check all luggage before bringing it to school to insure the proper type of clothing, toiletry, and clean snacks / lunch have been packed. −Please see suggested packing guidelines in the D.C. informational packet. We are relying upon parents to supervise the packing process to ensure students do not bring items that may violate the school’s Code of Conduct or dress code. We will not inspect luggage before our departure on Wednesday morning. However, we reserve the right to search bags as needed.

11 CARRY-ONS / LUNCH / SNACKS Suitcases will be stowed beneath the bus; therefore, STUDENTS WILL NOT HAVE ACCESS TO THEM. Anything needed during the day should be packed in the carry-on. (See guidelines) Wednesday’s lunches should be entirely disposable and free of glass bottles. Please remember to AVOID all peanut and milk products. All drinks must have caps. Snacks must also be peanut and dairy free. Students are responsible for cleaning up after themselves on the bus.

12 CELL PHONES / ELECTRONICS Cell Phone / electronic games are permitted. (Wi-Fi and plug-ins will be available on the bus.) −On the bus, phones are to be on vibe mode −Cell phones are to be turned off and put away whenever we have a tour guide on the bus talking and when touring at the various sites. −Phones will be taken away if seen at inappropriate times. If cell phones are used inappropriately, they will be confiscated and returned to a parent on Friday. If usage violates school policy, students will be disciplined. Cyber bullying, accessing or distributing obscene material, will be reported to and dealt with by the local Police Department and any other governing law enforcement agency. DVD’s, cameras, and all electronics must be labeled with your name and are brought at your own risk. Westlake City School District is not responsible for any lost, stolen, or damaged personal devices.

13 MEDICAL INFORMATION REQUIRED PAPERWORK FOR ALL STUDENTS: −Emergency Data Sheet −Signed the Authorization to Consent to Treatment of a Minor form. Please note all the medical information on the permission form we need to know including all allergies, chronic conditions, etc. ALL FORMS AND MEDICATIONS ARE DUE NO LATER THAN FRIDAY MAY 27 TH. Students who do not have the completed forms will not be permitted on the trip.

14 MEDICATION CONSENT FORM(S) A Staff member will carry and administer all medication except rescue medications: inhalers and Epi-Pens. Over the counter medications must be listed on the back side of this form and the form must be signed by a parent. If your student needs to take any prescription medication, you must complete the form below with the medication name, dosage and times to be administered. The parent and a physician MUST sign this form. This form must include if your student will carry and self-administer his/her inhaler and/or Epi-Pen. If your child has an allergy and carries an Epi-Pen you must also complete an Emergency Action Plan. All medications must be in original bottles or packages. The parent must deliver the medications and this completed form to the office by May 27 th. Students may not share medicine. If a student is caught distributing or taking a drug, local authorities will be contacted as per the board policy.
