2 HCI Implementation Grant Kansas Health Foundation will provide:  Funding Up to $25,000 per year over three years contingent upon a local cash match of $.50 on the dollar or $12,500 each year. Cash Match FAQ  Significant technical assistance To implement the community change framework to create an environment that supports the policy priority identified during the planning grant year.
