6 Objective of WH&S Management Plans
The objective of the Electrical Safety Management Plan is to ensure that an ‘electrically safe’ working environment is maintained on the site for the mutual benefit of a QNP Stakeholders including: employees, contractors, the public and customers Zero Incidents

7 Zero Incident Philosophy
In adopting the concept that all injuries are intolerable and preventable we must constantly strive for Zero Incidents. If an injury does occur we are obligated to identify how and why the injury happened and then make positive changes to work methods in an endeavour to avoid any repeat episodes. However, the goal of Zero Incidents remains our constant target.

Electrical Workers QNP and Contractors Electrical workers have clearly defined obligations under the Electrical Safety Regulations This Electrical Safety Plan is in place at QNP to assist all parties meet many of these obligations Electrical Contractors Representatives Electrical Contractors engaged on this site are in their own right responsible for compliance with the Electrical Safety Regulations thus providing a workplace that is electrically safe as defined in the Electrical Safety Regulations

14 All Employees All employees have a responsibility for their own Health and Safety and thus, are encouraged to PARTICIPATE in all aspects of this Electrical Safety Management plan, for their own safety and the safety of their co-workers All employees have the authority to stop work when it’s unsafe to continue

15 Fitness for work Every Employee shall be responsible for ensuring they are fit for work at the start and throughout each work period. This includes taking responsibility to ensure their fitness is not impaired through health reasons including the effects of fatigue, stress, alcohol or other drugs Any employee that believes that he/she is ‘NOT FIT’ for work and/or any particular given task, has a responsibility under the Work Health & Safety Act to report to his/her immediate Supervisor BEFORE proceeding Initial the Induction Check Sheet 

17 Clause 7.1 QNP Safety Procedures During all QNP operations the requirements of all applicable QNP Procedures and all applicable statutory legislation shall be met. The legislation generally covering this Electrical Safety Management Plan is : The Electrical Safety Act, Regulations & Codes of Practice The Workplace Health and Safety Act & Regulations

18 The Electrical Safety Act
You can meet your obligations under the Act by: Complying with the Electrical Safety Act & Regulations Complying with Ministerial Notices Complying with the Codes of Practice

19 Defences If charged with a breach of an obligation, you would be able to provide a valid defence by establishing that the offence was due to causes beyond your control. Where there was no regulation, code of practice or ministerial notice that told you how to meet your electrical safety obligation under the circumstances, you could provide a valid defence by establishing that you chose an appropriate way, took reasonable precautions and exercised proper diligence to discharge the safety obligation. You would not be able to claim as a defence sections 23 and 24 of the Criminal Code dealing with accidental acts or omissions, or a mistaken belief.

20 Penalties Failing to meet your obligations under the Act:
Maximum penalties If the breach caused multiple deaths: 2000 penalty units ($150,000 for an individual, $750,000 for a corporation) or three years’ jail. If the breach caused death or grievous bodily harm: 1000 penalty units ($75,000 for an individual, $375,000 for a corporation) or two years’ jail.

21 Penalties Failing to meet your obligations under the Act:
Maximum penalties If the breach caused bodily harm: 750 penalty units ($56,250 for an individual or $281,250 for a corporation) or one year’s jail. Otherwise: 500 penalty units ($37,500 for an individual or $187,500 for a corporation) or six months’ jail.

22 Safe Work Procedures This Electrical Safety Management Plan has been prepared to assist QNP, its employees and Contractors to jointly meet obligations under the Electrical Safety Act 2002 This Electrical Safety Management Plan is based on the risk management process outlined in the Code of Practice 2010 – Electrical Work. Therefore, for QNP, its employees and contractors to meet combined obligations under the Act, compliance with this Electrical Safety Management Plan is mandatory

23 Safe Work Procedures …2 Many of the Safe Work Procedures used by QNP employees relating to electrical safety compliance have been prepared as Generic Work Method Statements. 31 Work Method Statements (JSEA)
Clause 7.4 Work Method Statements (JSEA) A Work Method Statement (WMS) will be prepared where there is a risk of bodily harm, damage to plant or equipment. The task specific WMS will examine the safety needs and requirements in a pro-active manner. Depending on the complexity of a particular WMS the relevant people selected from engineering, production, safety department and the work crew may also be involved to develop it. All workers assigned to the task will review and sign onto the WMS prior to starting the work.

Principles To ensure Queensland Nitrates Pty Ltd (QNP) provide an ‘electrically safe’ workplace defined by the Electrical Safety Act 2002 To ensure direct employees of QNP meet their own obligations under the Electrical Safety Act in sustaining the ‘electrically safe’ workplace provided by QNP To ensure Contractors engaged by QNP meet their own obligations defined by the Electrical Safety Act whilst working on the QNP site

Responsibilities General Manager – is responsible for making sure that this Policy and associated Procedures are implemented as appropriate by those listed below and that an effectiveness review of this Policy is untaken regularly (at least every 12 months) Site Electrical Engineer – is generally the Person in Control of the electrical installation as defined by the Electrical Safety Act in most cases, unless that role is partially contracted to another party e.g. an Electrical Contractor Is responsible for ensuring that QNP employees and Contractor Representatives are trained on the use and implementation of this Policy and associated procedures

Responsibilities QNP Employees – are responsible for compliance with this Policy and associated procedures and/or any other direction given by the above in relation to Electrical Safety Contractors – are responsible for compliance with this Policy and associated procedures AS A MINIMUM. This Policy does not remove the obligation of a Contractor to provide an ‘electrically safe’ workplace for its employees and others in the vicinity of ‘electrical work’ being conducted by the Contractor on the QNP site.

43 Working Live Policy To ensure Queensland Nitrates Pty Ltd (QNP) provide an ‘electrically safe’ workplace defined by the Electrical Safety Act 2002 for the safety of QNP & Contractor employees and others that may enter the site Live electrical work (including testing) is not permitted is on the QNP site unless this policy is strictly adhered to. For the purpose of this Policy Live-Work is divided into two types: Fault Finding & Testing that can only be done live only when it complies with the requirements below All other instances of live work are expressly forbidden unless a Work Method Statement is prepared based on the Flow Chart shown at Appendix A of the Policy, then reviewed and approved by the Site Electrical Engineer or the General Manager

QNP Employee – is responsible for implementing the proposed risk controls detailed on the APPROVED work method statement prior to commencing any live work in accordance with the procedures noted below Contractors – are responsible for compliance with this Policy and associated procedures AS A MINIMUM. This Policy does not remove the obligation of a Contractor to provide an ‘electrically safe’ workplace for its employees and others in the vicinity of ‘electrical work’ being conducted by the Contractor on the QNP site

46 Included in the Electrical Safety Management Plan
Clause 8.1 Included in the Electrical Safety Management Plan QNP is an operational plant. As such, the requirements detailed in this Electrical Safety Management Plan relate to an operational plant and the electrical workers that maintain the plant either directly or via contracted services However, on the occasion that installation work is to be carried out by electrical contractors, particularly on larger scale works, some of the requirements detailed in this plan may have to be reviewed on a case-by-case basis. If in doubt about anything contained in this plan you must contact the Site Electrical Engineer before proceeding.

47 What is NOT included This plan does not include High Voltage installation or testing activities these are covered under QNP’s High Voltage Isolation and Access procedures

48 About Electrical Work at QNP
The majority of electrical work undertaken by QNP employees on the site is the performance of electrical testing, fault finding, repair work and minor repairs associated with the on-going maintenance of the plant It is the policy of QNP and requirement of the Electrical Safety Act that employees and contractors DO NOT carry out live work unless the work complies with QNP procedures and the requirements of the Act.

49 Working on Live Equipment
If live work is unavoidable the following requirements from the Electrical Safety Regulation 2002, Section 12, is strictly adhered to:

50 Working on Live Equipment … 2
The following circumstances are required for the performance of live work – it is not practicable to perform the electrical work other than by live work because of 1 or more of the following – it is necessary in the interests of safety, whether or not electrical safety, for the work to be performed while the electrical equipment the subject of the electrical work is energised; a supply of electricity is necessary for the proper performance of the electrical work; there is no reasonable alternative to performing the electrical work by live work;

Before live work can commence, the following conditions also apply: QNP and/or Contractor has prepared a written risk assessment of the performance of the live work (including testing and fault finding); the Site Electrical Engineer or General Manager authorises the performance of the live work; the employee who performs the live work has appropriate qualifications and training for the performance of the live work; testing equipment appropriate to the performance of the live work has been given to the employee performing the electrical work, the testing equipment has been properly maintained, and the employee performing the electrical work makes proper use of the testing equipment in performing the work;

Before live work can commence, the following conditions also apply: clothing and personal protective equipment appropriate to the performance of the live work has been given to the employee performing the electrical work and the employee performing the electrical work makes proper use of the clothing and equipment in performing the work; the isolation point of the electricity supply for the electrical equipment the subject of the electrical work has been clearly identified and, except for electric line work, is able to be reached quickly without the need to climb over or shift obstructions;

53 Working on Live Equipment … 5
Before live work can commence, the following conditions also apply: the area where the electrical work is performed is clear of obstructions to the extent necessary for easy access to and from the area; there is a safety observer observing the performance of the electrical work, unless the work involves testing or fault finding of electrical equipment; the performance of the electrical work is in accordance with a safe system of work.

54 Meaning of a Safe System of Work
Safe system of work, for live work on a low voltage electrical installation, includes, but is not limited to, a system of work that complies with the provisions of AS-4836 (Safe working on low voltage electrical installations) about ensuring the safety of persons while performing live work.

55 Meaning of a Safety Observer
Safety observer, in relation to the observing of the performance of electrical work, means a person – who is competent to help with the electrical work; and who is competent to rescue the person performing the electrical work and to provide resuscitation; and whose competence for paragraph (b) has been assessed in the last 6 months by QNP or Contractor.

56 Clause 8.7 Testing Live There are times when Live Testing is necessary; when fault finding, polarity testing etc. Live testing can only be carried out by employees that meet the following criteria: Have completed an Electrical Refresher course approved by QNP and have their names listed on the Register for Work Permit System Authorisations Have been instructed by QNP to perform the tests Comply with the Selection and Safe Use of Low Voltage Test Equipment - Procedure that includes: testing the equipment is working properly before (and after) the tests using equipment with a current calibration certificate

57 Testing Live … 2 Have taken all necessary steps to make sure they are electrically safe before testing commences Have remove/cover any Jewellery before testing commences, see Jewellery clause below Where the correct PPE, including the appropriate gloves, referring where necessary to the Arc Flash Protection Matrix Review the WMS before starting to make sure all risks have been properly controlled to a low – negligible residual risk Use a safety observer when the risk of electrical contact is above low – negligible e.g. when testing in a busbar chamber of a MCC as apposed to checking the polarity of a GPO Properly record the test results and submit in a timely manner to QNP for filing as per the Testing for Electrical Safety Procedure

58 Low Voltage Isolating and Energisation
The only QNP employees that are authorised to carry out low voltage isolating and energisation functions are those nominated and deemed competent by the Site Electrical Engineer and names listed on the Register for Work Permit System Authorisations. These employees may/may not be licensed electrical workers providing they have been properly trained and deemed competent to do the function in an electrically safe manner. The Register for Work Permit System Authorisations is maintained by the Safety Coordinator.

59 Non-electrical or untrained electrical personnel
Where non-electrical or untrained electrical personnel are required to work adjacent to exposed live equipment (including testing), their work and actions need to be closely supervised by trained persons at all times. Prior to any work commencing near exposed live electrical parts (other than testing or fault finding), Work Clearance Recipient must conduct pre-start meeting and run through the ‘approved’ Work Method Statement to ensure all participants, trained and non trained are completely sure of their individual roles, responsibilities and obligations. The following matrix is an extract of pertinent Exclusion Zones – from Appendix C of the Code of Practice 2010 – Electrical Work.

Usually this is the General Manager or in some cases it could be the Site Electrical Engineer and/or Contractor

61 Jewellery The wearing of jewellery, and the associated hazards, must be considered when carrying out WMS for tasks. Examples from the Code of Practice 2010 – Electrical Work, Section of metal objects that should not be worn when performing electrical work include: Neck chains, Rings, Bracelets, Earrings, body piercing, and Metal spectacle frames.

62 Pre-start Safety Talks
Whenever there is a requirement to carry out live work, other than routine testing/fault finding, the Work Clearance Recipient must conduct a pre-start safety meeting to discuss the planned activities and Work Method Statement with the entire team to ensure all participants understand their roles and responsibilities. You will be required to sign a Pre-Start attendance register Check you PPE and Test Equipment before the Talk Initial the Induction Check Sheet 

64 Other Site Requirements
Section 08

65 Clause 9.1 Barricades It is the responsibility of the work group creating a hazard to erect and maintain barricades as required for the protection of personnel. Orange Barricading shall be used for floor openings, stairwells, platforms and walkways, or trenching where a person can fall any distance. An appropriate sign and information tag must also be attached to the barricade on all sides where entry to the area is likely.

66 Barricades Yellow and Black Barricade Tape will be used to prevent personnel entering a Hazardous area where risk of injury is high eg. People working above, when cranes or lifting equipment are in use or during electrical testing etc. All Barricades will be erected at a height of 1100mm, be adequately supported and clearly visible to all persons approaching from any direction. The reason for the barrier should be attached by means of a tag, label or sign.

67 Barricades It is the responsibility of the person creating the hazard to erect and maintain barricades as required for the protection of workers and personnel. The person/team that erects a Barricade MUST also ensure the Barricade is removed immediately upon completion of the works. Initial the Induction Check Sheet 

69 Safety Tag System All isolation, tagging and lockout will be done strictly in accordance with the QNP Plant Lockout and Safety Tag System detailed in the Work Clearances Manual. Initial the Induction Check Sheet 

70 Personal Protective Equipment
Section 10

71 PPE QNP will provide appropriate personal protective equipment for QNP employees in all work situations. Contractor PPE will be provided by the Contractor’s employer PPE provided will be as per company procedure, and/or as determined by any risk assessment and nominated on a Work Method Statement

72 General PPE requirements
Look, on an on-going basis, for more effective control measures to reduce the need for PPE Only used approved PPE, do not bring other types of PPE to the site Visually check the condition of the PPE before each shift, prior to each use, or if you suspect it may be have been damaged Defective PPE shall be disposed of properly, replaced, or repaired by qualified personnel Employees of subcontractors and visitors shall comply with these PPE requirements The PPE supplied will comply with all appropriate Australian Standards and is to be used in accordance with manufacturer’s instructions and/or as instructed by qualified persons

73 Minimum PPE Requirements
Safety glasses Hard Hat Safety footwear Long trousers and long sleeve shirt Gloves suitable for the type of work being undertaken for work on or in proximity to live parts. When working in areas where the noise level exceeds 85 dB, hearing protection will be worn.

74 PPE Summary Prior to each working day, and before each time you use it, you must inspect your PPE and confirm its condition. You will be provided with instruction on other PPE such as harnesses and lanyards etc. when appropriate If you have not received instruction on PPE do not use it until your Supervisor has provided you specific instructions for its safe use Initial the Induction Check Sheet 

78 Clause 9.5 Welding Only qualified and tested welders can perform welding duties on this site. Only those employees that have welding duties included in their site job description will be authorised to perform welding. Clause 9.6 Confined Space Procedure QNP will provide training for all persons required to work within a CONFINED SPACE as defined in AS-2865 Safe Working in a Confined Space Do not work in confined Spaces if you have not had this training

84 Review of this WH&S Management Plan
QNP will conduct a complete review of Electrical Safety Management Policies and Procedures every 12 months. The purpose of the review; to evaluate its effectiveness and, to ensure the evolution of the site is embraced within the plan. The Site Electrical Engineer shall ensure any changes to this plan will be explained to employees in a timely manner via Toolbox meetings or other appropriate means.
