4 This folder will be located in the FORMS notebook. It will contain all of the signs & forms needed at your station. On the back of this folder is a Ballot Station Procedures checklist. It will walk you through set-up, procedures during Election Day, and closing your station. BALLOT STATION FOLDER

5 BALLOT STATION FORMS: BALLOT ACCOUNTING FORM Count and account for ALL ballots: Voted, Spoiled, Unused, and Challenged. BALLOT LOG Count & record ballot numbers as sets of ballots are retrieved from the Un-Voted ballot bag.

6 BALLOT STATION PRE-ELECTION SETUP: 1.Obtain Ballot Station folder and supplies from the Chief Judge. 2.Post signs and organize supplies. 3.Review Forms. 4.Practice setting up the Ballot Style Barcode/Ballot Scanner. Ensure you have access to an outlet to charge the scanner battery on Election Day. REMEMBER to remove the scanner battery before you leave for the night!

9 VERIFY BALLOT STYLES AND NUMBERS BALLOT STUB numbers are recorded on the Ballot Log when taken out of the ballot bag. Record the beginning & ending number of each set throughout the day. ELECTION DAY MORNING BEFORE 6:30 AM Stub Number 101

11 USING THE BARCODE SCANNER: AFTER 6:30AM Using the scanner instruction sheet: 1.REVIEW THE ATV FOR OFFICIALS INITIALS, VOTER SIGNATURE, AND BALLOT STYLE. 3.PULL THE CORRECT BALLOT STYLE. 4.SCAN THE BARCODE AT THE TOP RIGHT HAND CORNER OF THE BALLOT. 5.SCAN THE BALLOT STYLE ON THE ATV FORM 6.LISTEN FOR A ‘GOOD’ BEEP AND VERIFY THE SCANNER SCREEN DOES NOT SAY MISMATCH. 7.PLACE THE VOTER’S BALLOT IN A SECRECY FOLDER, GIVING ANY INSTRUCTIONS NECESSARY.  IF SCANNER SAID MISMATCH; CHECK THE BALLOT STYLE PULLED AND SCAN BOTH DOCUMENTS AGAIN. REMEMBER TO LISTEN FOR THE BEEP!

12 ATV FORMS Voters will arrive at the Ballot Station with a Authorization to Vote (ATV) form. They must have this form in order to receive a ballot. Inspect the form for signature, initials, and ballot style. The ATV forms will be numbered and stored in a three ring binder at the Ballot Station throughout the day.

13 WORKING WITH ATVS: Check the following 3 items on ALL ATV Forms: Voter Signature in Section A. Officials Initials in Section A. Verify and Issue Correct Ballot According to Style Label

14 CURBSIDE VOTERS When the Help Station Official brings you an ATV form, they are assisting a curbside voter. This ATV form will not have initials or a signature when the ballot is issued. Issue correct ballot and ATV form in secrecy folder to the Help official. The Help official will return the completed ATV form to the Ballot Station when the voter has completed the ballot.

YES--Curbside Voter 1.Issue the ballot indicated by the Ballot Style. 2.Verify by scanning the BALLOT BARCODE on the top right corner of the ballot and the BALLOT STYLE BARCODE on the ATV. 3.Issue the ballot and ATV form in a secrecy folder to the official.

21 BALLOT STATION – SCENARIO #3 CURBSIDE VOTER - COMPLETING THE PROCESS 1.The curbside official will return the ATV with sections A & D completed. 2.Number the ATV and place in the completed ATV Notebook. 1

24 BALLOT STATION - BALANCING THINGS OUT: 1.AT DESIGNATED TIMES DURING THE DAY (10:00 A.M., 2:00 P.M., 4:00 P.M., & 7:30 P.M.) THE CHIEF JUDGE WILL ASK YOU FOR THE NUMBER ON THE MOST RECENT ATV ISSUED. 2.THIS NUMBER SHOULD EQUAL THE NUMBER OF BALLOTS IN THE DS200 + EMERGENCY BIN + VOTERS IN BOOTHS. 3.IF THIS DOES NOT MATCH, MARK THE ATV AS A POINT OF REFERERENCE. 4.INSPECT THE ATVS FOR DUPLICATE NUMBERS OR MISSING NUMBERS. 5.DO A COUNT OF UNUSED & SPOILED BALLOTS.

28 Scan the ballot style barcode on top of the BALLOT LOOK AT THE SCANNER Scan the ballot style barcode in Section A of the ATV LOOK AT THE SCANNER Listen for the ‘GOOD’ beeps

29 1.Count your ballots and check numbers and styles with the Ballot Accounting Sheet and Chain of Custody Form. 2.Complete Ballot Log each time a stub of ballots is removed from the bag. 3.Ensure the barcode scanner is set up and ready to use.

32 Process the ATV given to you as if it were given by a voter on Election Day. Decide if you would issue a ballot.

33 ISSUE A BALLOT D003 Be sure that you have checked all three requirements before issuing a ballot. Scan the barcode on the Ballot then scan the Ballot Style Barcode on the ATV, making sure they match. Issue the ballot to the voter in a secrecy folder. Number the ATV. 1

34 Send to Voter Check-in Station The ATV does not have the voter’s signature. DO NOT number the ATV.

35 A Precinct Official brought you this ATV; it is a curbside voter. Scan the barcode on the Ballot and then scan the Ballot Style Barcode on the ATV, making sure they match. Place the ballot and ATV form in a secrecy folder and give to Precinct Official. DO NOT NUMBER ATV. ISSUE A BALLOT R001

36 Curbside Voter Continued Look for voter signatures and official’s initials. If voter signatures are missing send official back to voter to get signatures in sections A & D. Number the ATV and place in the completed ATV Forms Notebook. 2

37 Direct the voter to the Help Station. This is the form that is generated by the Voter Check-in Station to refer the voter to the Help Station. DO NOT issue this voter a ballot. They should have gone to the Help Station.

38 ISSUE A BALLOT D003 Be sure that you have checked all three requirements before issuing a ballot. Scan the barcode on the Ballot then scan the Ballot Style Barcode on the ATV, making sure they match. Issue the ballot to the voter in a secrecy folder. Number the ATV. 3

39 Send to Voter Check-in Station The ATV does not have the voter’s signature or initials in Section A. DO NOT number the ATV.

40 CLOSING THE POLLS – BALLOT STATION: USING YOUR CHECKLIST COMPLETE THE FOLLOWING: 1.SIGN THE BACK OF THE LAST ATV ISSUED (ALONG WITH THE CJ AND J OF ANOTHER PARTY). 2.SIGN THE BALLOT LOG 3.COMPLETE AND SIGN THE BALLOT ACCOUNTING FORM 4.SEAL THE SPOILED BALLOT BAG, EVEN IF EMPTY AND PLACE IN THE UNVOTED BALLOT BAG. 5.LOCK THE UNVOTED BALLOT BAG. 6.PLACE ALL SIGNS AND FORMS IN THE STATION FOLDER. (SIGNED FORMS ON TOP) 7.RETURN BARCODE SCANNER, ATV FORMS IN NOTEBOOK, SECRECY FOLDERS, AND PENS TO THE BLACK SUPPLY BAG. 8.ASSIST CHIEF JUDGE WITH OTHER TASKS AS REQUESTED SPOILED

41 The number on the LAST ATV Form issued should match the number of ballots in the DS200 tabulator. This should be the total number of Voted Ballots. Use the DS200 tabulator tape to record the number of voted ballots, by ballot style on Line D of the Ballot Accounting Form. Count the ballots, by style, in your blue unused ballot bag and any that are still on your table. This number is recorded on line A. Count the ballots in your clear Spoiled Ballots Bag and record, by style on Line B. If you have any Challenged Ballots, they are to be recorded on Line C. Add all numbers in white boxes down the columns (Lines A through D) and across the rows. Record totals in yellow boxes. Add yellow boxes and record in the pink boxes. The pink boxes should be the same. If not, you must explain why the totals do not match. Sign both logs, place in Ballot Station folder and give folder to the Chief Judge. WHERE THE NUMBERS COME FROM:

42 Polls are closed and it’s time to complete the Ballot Accounting Form. Count your unused ballots. Are there any spoiled ballots in the spoiled ballot bag? NO Are there any challenged ballots? NO Get the DS200 count from the Chief Judge. 1 R001 and 2 D003 Get the Hand Count Number from the Chief Judge. Zero Add the totals for each column and each row. Make sure that pink boxes are equal. If not explain why they do not match.

44 BALLOT STATION PROCEDURES & REMINDERS: ONLY ISSUE A BALLOT IF THE ATV IS PROPERLY COMPLETED WITH: 1.OFFICIALS INITIALS 2.VOTER SIGNATURE ATV’s are numbered sequentially. Never write on a ballot! A VOTER MAY SPOIL UP TO THREE BALLOTS. DO NOT PRE-STUFF BALLOTS IN SECRECY FOLDERS! YOU MUST COUNT ALL BALLOTS. IF THERE IS A DISCREPANCY ASK FOR ASSISTANCE. IF YOU ARE UNABLE TO FIND THE ERROR, NOTE THE DIFFERENCE. DO NOT SUBTRACT TO MAKE THE NUMBERS WORK!
