ORGANIZATION OF TEST MATERIALS Follow instructions beginning in Appendix C (page 100) ONLY for testing materials to be shipped (FSA provided labels) – TO BE SCORED – regular print test and answer books – TO BE SCORED – large print test materials – TO BE SCORED – Braille test books – NOT TO BE SCORED materials - includes all unused regular print test and answer books, special documents, passage booklets, and used test and answer books that should not be processed and scored. – DISTRICT COORDINATORS ONLY materials –DO NOT FOLLOW the directions on page 106. Package as per box/envelope labels 2

SHIPPING LABELS FOR TEST MATERIALS 3 It is your responsibility to package all materials correctly. Miss-packaged materials will impact the reporting of student results. * Striped labels for calibration materials will be the same color as the applicable administration but will include a stripe across the middle of the label.

ORGANIZATION OF TEST MATERIALS CON’T….. – TO BE SCORED MATERIALS Boxed according to chart for label colors – Place in plastic bags provided by DRC – Close boxes with “A” and “B” flaps exposed – Place the correct GREEN*, WHITE, or PINK shipping label on flap A – Number Boxes by color of label n of n *Calibration schools for WRITING and Calibration schools for READING will both receive GREEN labels with a STRIPE in the middle. RETURNING TEST MATERIALS KEEP ALL DISTRICT COORDINATOR MATERIALS AT YOUR SCHOOL FOR THE MAKE- UP ADMINISTRATIONS You will organize materials within the boxes by administration date Labels and/or Boxes for Manuals will be distributed at a later date Date for return of District Coordinator materials will be announced at a later date 9

RETURN RECEIPT: RETURN ONLY THE WRITING TEST MATERIALS THAT WILL BE SHIPPED TO VENDOR Fill in the number of tests returned for scoring in the blocks provided Complete BOTH copies and return with testing materials to EVALUATION SERVICES ON MONDAY MARCH 07, 2016 BETWEEN 8:30 AM AND 2:00 PM 10SPRING 2016 FSA WRITING RETURN
