12. To summarize… Where we live matters to our health. There are great disparities in health based on where we live. Health is more than health care—many factors contribute to health. We’re all in this together—we all need to work together to improve the health of our community.

13. An underlying belief… We believe we can become a community where getting healthy, staying healthy and making sure our children grow up healthy are top priorities. We have a vision of a community where we all work together—looking at the factors that influence health, focus on strategies that will work and make changes that will have a lasting impact on health– to enable all to lead healthy lives, now an for generations to come.
