3 Friday April 7, 2006 J. Anderson Jr. Page 3 ES&H Radiation Exposure  ~5.2 R total dose for the first 5 weeks of the shutdown.  Reminder to get weekly dose cards or electronic dose recording turned in on Monday’s.

Anderson Jr. Page 4 ES&H One injury in the past week where an employee twisted their right ankle in a floor depression that resulted in an ankle fracture.  Direct cause was the floor depression.  Two contributing causes were: Warning cones originally used to mark the poor surface conditions were moved to facilitate equipment movement through the area and were not replaced in a timely manner.  The work plan for the equipment movement should have considered getting the cones put back in place. Second was the employee unaware of the poor surface conditions.  The employee was talking to a co-worker at the time of the incident and they were not focused on the conditions around them.
