1 Electron Gun Group Laser Safety Review September 8, 2005 Agenda 5 year history of DOE lab laser safety reportable incidents Upcoming DOE laser safety review Review laser safety for Injector, ITS Laser Room and ITS South Cave Discussion - How can we improve laser safety in our work areas?

2 Laser Safety Officer Workshop, Lawrence Livermore National Lab, NIF August 3-5, 2005 6 laser worker eye injuries since 2001 at DOE laboratories No eyewear during any of these injuries 4 of the 6 injured were STUDENTS 5 involved use of Class 4 laser (like our Ti:Sap) 1 involved use of Class 3b laser (like our diode) Injuries result from poor laser training and hazard controls Integrated Safety Management => Event Root Causes Training -> weakness in training, lack of compliance with codes LSO/LSS ->authority and role model for laser safety Internal Oversight -> inadequate line management Personal Protective Equipment (PPD) -> absolute enforcement

3 Audit Finding: Leading indicator of good laser safety is good “Housekeeping”.

6 September 13 th -14 th, by the Oak Ridge Site Office A list of LSS’s and users will be provided Auditors will pick randomly to meet LSS and Laser Workers Upcoming DOE Laser Audit Jlab In-House Safety Assessment, May 2005 Agenda set by DOE and ANSI code standards Findings 1.LSO approves wording on laser signage 2.Formally designate the deputy LSO: John Kelly 3.Formal designation and documentation of LSS quals 4.Formalize procurement process for purchase of lasers 5.Review DOE eye injuries and root causes with Laser Workers 6.Evaluate and implement refresher training where necessary (LSO) 7.Formalize laser specific training, must indicate depth and scope of on the job training

7 Review DOE lessons learned Demonstrate that safe work is happening: –Know procedures –CLEAN UP!!!! Establish clear roles, responsibility, and authority for laser operations Demonstrate active oversight of all aspects of laser Operations Monitoring and evaluating mentors and LSSs that oversee students Establishing and clearly explaining personal accountability, including reprimand policy for employees that do not meet safety performance specifications How can we prepare for the audit? The DOE agenda:

8 –Alignment training and review –Formal mentorship of students –Good Housekeeping –PPE planning and adherence Issues for Jlab and the EGG Laser Work Areas Open discussion of “How can we do better?” –PPE at all times –Prescription eye-wear in lieu of goggles atop goggles –NEW for eye injuries => victim should be transported “sitting up” to minimize further damage to the eye. –PPE planning and adherence
