Student Internship Placement and Required Forms

Once you have approval from your supervising faculty member, follow these steps to finalize the internship placement.

Using the Internship and Service Learning Database 1.Identify your site – Review database for possible sites – Select 2 or 3 sites to schedule interviews 1.Make your placement – Once you have gotten approval from faculty and a site, you can make your placement.

If your site supervisor is not listed, let us know and that name can be added to the database. internships@csusm.edu internships@csusm.edu

1. Select the correct term/year in dropdown box 2. Provide an estimate of the number of hours you will serve (You can change this number later on if necessary)

Placement Forms Click on “Complete this form online” Your placement is not complete until you have filled out all required webforms

Information to complete Placement Forms: Your Site Supervisor's Name and Email Address. Your Instructor's Email Address. Emergency Contact Name and Phone Number (this information will be made available to your internship site). Learning objectives (responsibilities and duties you will have while serving at the site that and what you are expected to learn). Agency Approval

Finishing Placement Click “Finish Placement” after filling out all of your information on placement form Make sure to click “View your submission” to make sure your information is correct

Click on “View your submission” to make sure your placement is correct.

What to expect once you’re placement is complete. All students will be added to the “Office of Internships” course container at Log on using your student user name and password. View the “orientation and workplace safety” video and take a short true/false quiz to demonstrate your understanding.

Some students may also need to upload the following: A signed release of liability form--needed if you opt to intern at a site that is NOT in our database; faculty approval is required. A signed release of liability form if you are under 18 years of age. A copy of your completed time sheet, signed by your site supervisor—needed only if your are interning at a site where you are also currently employed. Your faculty may also require all students to submit a time sheet her/him.

Tips Get started early Consult with your faculty supervisor about department expectations and requirements Find a few sites that interest you Contact community partners Consider site requirements (background checks, orientations)
