4 WWW.CHRODIS.EU Background Chronic Disease burden in Europe requires concerted action European reflection process on chronic diseases: Large pool of chronic diseases prevention and health promotion good practices exists across the EU, but the knowledge is not always disseminated and accessible to the best possible extend Good Practice approach within the Joint Action CHRODIS is complementary to the national activities against chronic diseases
