2 Objectives To be able to write a measurable AIM statement. Understand how a charter will guide your work

3 How to begin? With any improvement project you will need to know what you are trying to do or improve upon. An improvement project will need to have a document for organizing your work.

4 Which document to use There are MANY types of documents [Charters] that can help organize your work.

6 AIM statements Aim statements do the following: Define the problem The process you are trying to improve Articulate a clear message to the team The more specific the AIM the better chances the teams will be able to improve

7 AIM Statements Align Make the target for improvement bold and unambiguous Include deadline What is needed to keep the team focused

11 Components of Aim Statement Direction Boundaries/Process Measure Timeline Increase/Decrease System to be improved (scope, patient population, processes to be addressed) Defines what success looks like (How good?) Ensures an urgency to continue (By when?)

13 AIM statement examples Through the implementation of an EMR our patients will get better. The management of diabetes patients served at Make Believe Clinic will have their health outcomes improve. By May 2013 we will increase the percentage of patients meeting A1C and blood pressure targets from 25% to 70%.

14 AIM statement examples By the creation of an interdisciplinary team we will improve specialized cancer care and access by 25%. The Get Better Outpatient Clinic will reduce patient wait time for all renal consultations from 50 days to 25 days by July 1 st, 2013.

15 AIM statement examples Increase the number of AMI patients who receive thrombolytics within 30 minutes of hospital arrival to 95% by March, 2013. Increase the number of AMI patients receiving aspirin on arrival to 95% by March 2013.

16 Your Turn At your table there are AIM statement which could be missing information, misleading or are brilliant. Work in partners to re-write the AIM statements; be prepared to share and discuss.

17 Your AIM statements Spend the last 15 minutes thinking of your project and write your AIM statements.
