5 Number of chronic disorders by age group
The number of morbidities and the proportion of people with multimorbidity increased substantially with age. By age 50 years, half of the population had at least one morbidity, and by age 65 years most were multimorbid. 7 Need for Active and Healthy Ageing

8 Transformation of Health/Care Systems
Acute care Long-term-care Single Diseases Multiple chronic conditions Passive Patient Active consumer Institutional care Home care Generic Approach Personalised Care

9 A sizeable EU Silver Economy
3000 B€ Wealth by people over 65 85 Million Consumers over 65 and growing <15% ICT use The Consumer Market Health & Care costs ~1000 B€ /year in Europe (8 % GDP) 10% for innovation ~100 B€ /year The Public Market People over 65 (over 55) are wealthy and will be so also in the future The markets for ICT products and services and the markets for ageing well are still growing Care costs are growing and so are the markets for cost reduction and innovation: if 10% of care budgets was spend on innovation, we wouldhave a market of around € 100 billion per uear.

16 EIP on Active & Healthy Ageing
Reference Sites - coverage City of Oulu Northern Netherlands Twente Province of Gelderland and Overjssel South Holland Province Noord-Brabant: Slimmer Leven Region Skane Southern Denmark Liverpool Scotland Northern Ireland Wales Yorkshire Collage (3) Saxony University Hospital Olomouc The EIP's Reference Sites constitute the second parallel track in the work of the Partnership which aims to identify and spread excellence in practice at local level. The 32 Reference Sites were selected through a peer-reviewed process from the applicant regions, cities and integrated hospitals/care organisations. They take a comprehensive, innovation-based approach to active and healthy ageing, with evidence and concrete illustrations of their social and economic impact on the ground which can be transferred to other European contexts. The Reference Sites have developed a strategy for scaling up, giving an overview of the success factors, lessons learned and transferable elements of their good practices. 17 EIP-AHA Results so far A number of proven innovation guidelines and toolkits Eg Fall prevention, Integrated Care A repository of more than 250 proven innovation practices A common tool and outcome indicators to aggregate socio-economic evidence from AHA innovation Access to more than 500 innovation partners across Europe 17

18 Northern Ireland 'Integrated Medicines Management 150,000 people currently receive the service with over 97,500 being 65 or over Reduced length of stay (2 days - 20% decrease) Return on Investment £5 for every £1 invested Pharmaceutical Clinical Investing Programme Savings £140 million (25% of prescribing Budget) Improved medicines appropriateness index(17.48 to 5.69) Reduced medication errors at admission (4.2) Reduced errors at discharge (20%-<1%)

19 x EIP-AHA Outcome Indicators Headline Target Triple Win
+2 HLYs Triple Win Quality of Life Sustainability Innovation & Growth HRQoL Mortality Incremental change in resources used Nr. of implemented technologies Nr. of users of new technologies Outcome Indicators on intervention / commitment level x Risk factors Physical Activity Nr. of created jobs Nr. of new SMEs Adherence Frailty (Local) unit cost for resources Speaker: Peter Wintlev-Jensen HrQoL and Mortality are primary indicators (i.e. general enough to be relevant across all Action Groups of the EIP on AHA, but also sufficiently specific and sensitive to capture the impact of particular interventions within each Action Group) Greyed indicators are Secondary indicators (i.e. they require a more elaborate approach for linking outcomes to the EIP on AHA objectives and the headline target and they could either be relevant for several Action Groups, or targeted at particular Action Groups) Functional status Falls Some frequent resource categories: Nutrition Mental health Hospital (re-) admissions Length of hospital stay Emergency visits Primary care visits Specialist visits Institutionalisation Cognitive decline

20 Next Steps for the EIP-AHA
Scaling up implementation Impact assessment metrics & tools Providing a critical mass of impact evidence

21 Next Steps for the EIP-AHA
New call for commitments New call for reference sites Expansion Revised action plans Focus of near term outputs Operational focus December 9-10th 2015 Reinforced Political support Launch of proven practice repository Conference of Partners

22 Hvorfor deltage i EIP-AHA?

23 Hvordan deltage i Horizon 2020?
