Patient Navigator Establish trust & rapport with the family. Establishes relationship. Connects the family to those resources.

Center for Diversity and Health Equity Comparison to a Care Coordinator Care Coordinator Arranges access to services. Promotes continuity of care in the community. Supports family during hospitalization and transition to home. Connects family to resources. Patient Navigator Educates families how to use the services. Follows up to assure that care is occurring as planned. Evaluates appropriateness and coverage of support. Assures the family can use the resources.
