1. IRP Review You will be tasked 30 days before you review is due, and it MUST be complete with in the moth it is due for services to be billable. You can click on Resolve from your task list, or you can go into the IRP and click the Review button All forms that have a Red Circle and X must be completed by the therapist. Click add to add a new form. Once the form has been added the Red Circle will turn to Green check mark.

2. IRP Review If a client has more than one Goal, a Primary Goal, a Case Management Goal AND/OR a SDS Goal, the CM goal and Objectives will need to be reviewed by the Case manager, the SDS goal will need to be reviewed by the SDS worker and the Primary Therapist will review their own Goal and Objectives.

3. Click on the Plus mark (+) next to the goal. This will show you the objectives attached to the goal. Please write a review for each of the objectives. Click the Save button when you are done. The red X will turn to a Green check mark IRP Review

4. Once all of the Goals have been reviewed by the Case manager, SDS worker and Therapist, the Review MUST be signed by the Primary Therapist. Once signed and saved, the review date will advance to the next 3 or 6 month review date.

5. Discontinue Goals To discontinue a Goal, click on Discontinue. Before you can discontinue a goal, it will need to be reviewed. The best time to discontinue a goal is during the review process.

6. Add a Review for each of the objectives, enter the Discontinue date, any comments and the reason. Click the Discontinue Goal button, this will discontinue the goal. Do not click save or sign & save. Discontinue Goal

7. Update IRP Objectives After the review process it might be necessary to update the objectives. To update a Objective, click on up date. Go to the objective section and click Edit or Remove Objectives, Add new objectives or remove any completed ones. Sign and save form when you are done.
