Application Requirements  Application deadline: December 2, 2015 (to Lisa)  Students must have Minimum of 80% average in their previous degree, or Minimum of 80% average in the Fall 2015 term (at least two courses) A good command of English  Students may be interviewed by the department.  Any interviews will take place before the end of December (Will be notified by your waterloo email). 3

Co-op degree requirements  Students must complete two work terms.  Students must finish their program on an academic term, and maintain full-time status until completion.  The co-op placements must related to the program of study.  Co-op students should follow the regulations and procedures of Co-op Education and Career Action. http://uwaterloo.ca/co-operative-education/ 4

5. Co-op degree requirements  Students should submit a work-term report after completion of each co-op term. It will be evaluated by the department. If you fail the report, you won’t get the co-op designation.  Work-term report It is due no later than the 10 th day of classes in the academic term following the work term. Graduate co-op fees  A co-op fee will be assessed by Student Accounts (4- month co-op term).  Co-op students are not assessed Dental, Supplemental Health or GSA fees.  Students may enrol them through the GSA office.  International co-op students are charged for UHIP, but must self-enrol for dental and supplemental health coverage. http://www.adm.uwaterloo.ca/infofin/students/stdfees.htm 8
