1 Population Community Presentation Lauren Studdard July 24, 2013

2 The Population Group of 20-25 individuals who visit a local church’s “Kindness Ministry” on a weekly basis to obtain groceries and other necessities. The Kindness Ministry began in 2011 at a local church in Eclectic, AL. Once a week, individuals go pick up groceries and other necessities (soap, toothpaste, etc.) for free.

3 Population Goal To assess the population’s educational level and how it influences their overall health by participating in group discussions, while educating them on health prevention and promotion strategies regarding diet and exercise.

4 Objectives To perform an appropriate focused assessment in order to identify the population’s overall health. To identify their education level and readiness to learn, while providing education that meets their level of comprehension.

5 Objectives To educate them on health prevention and promotion using both printed handouts and technology through a PowerPoint presentation, as they demonstrate what they have learned through guided discussions and communicating understanding.

6 Survey The group completed a questionnaire with the following components: Income and Insurance Education level Availability of transportation Current diet and activity level Health problems/diseases Knowledge of healthy food choices

7 Results of Survey Health Disparities revealed Prevalence of diabetes and heart disease in the group Noncompliance with diet and exercise Lack of knowledge on the appropriate diets for diabetes and heart disease.

8 Health Disparities Lack of education Low income Lack of insurance Limited access to appropriate resources due to lack of transportation (doctor’s office, clinics, etc.)

9 Strategies Provide a PowerPoint presentation on health prevention and promotion strategies in regards to diet and exercise. Assist them in selecting appropriate food choices. Educate them on making healthy choices as they pick up their groceries. Use Healthy People 2020 as educational tool

10 Strategies Provide printed handouts with the following education: Relationship between health prevention/promotion with food and exercise. Healthy food choices Recommendations for exercise Effects of diet and exercise on heart disease and diabetes.

11 Healthy People 2020 Diabetes Goal: Reduce the disease and economic burden of diabetes and improve the quality of life for all persons who have or are at risk for diabetes. Heart Disease Goal: Improve cardiovascular health and quality of life through prevention, detection, and treatment of risk factors for heart attack; early identification and treatment of heart attacks; and prevention of repeat cardiovascular events.

12 The Discussion A guided discussion was used to evaluate outcomes. Key Points of Discussion: Appropriate food choices Ideas for increasing activity level/exercise Relationship between diet and exercise on heart disease and diabetes.
