Improved communication between healthcare professionals involved in the care and management of their condition An increased amount of information available and in a way which is easy to understand Improved coordination of care irrespective of where they live. Project aims & objectives... Consultation with Patient support Groups Gaps in service provision identified Consultations with member groups who have established Networks Clinicians with interest in each subset approached Successful strategies facilitating the creation of Networks are identified.
