The UW-Madison SHIP office is responsible for collecting all F-1 and J-1 visa holders’ insurance information The SHIP Office Prescriptions

UW-Madison Health Insurance Mandates Non-immigrant visa holders and their visa dependents must comply with the UW-Madison insurance requirements which meet or exceed the Federal and Wisconsin State insurance requirements UW-Madison requires that all non-immigrant visa holders and their visa dependents holders purchase SHIP or waive with qualifying insurance No travel insurance plans or private policies will be accepted

Automatic Waivers Paid position with the University of Wisconsin Madison (teaching assistantship, research assistant position, or a project assistant position) Coverage must take effect on or before September 1 st of this year Must Meet the Following Criteria:

After Enrollment You will receive a SHIP membership ID card by mail Keep your membership card with you at all times Card used for medical appointments and pharmacy visits

How to use SHIP: 1. Must use University Health Services for all available primary and preventive care services 2. “ In-Network” providers are available for instances when UHS is closed or unable to provide necessary services 3. Click the “Find ‘In-Network’ Providers” link on the SHIP website and contact our office if you experience any difficulties

SHIP Coverage and Costs In- Network Out-Of- Network SHIP Pays 100% Except for 3 rd party requested physicals and labs You pay 10% SHIP Pays 90% You Pay $500 deductible +40% SHIP Pays 60%
