9 With the exception of the amount requested, all of the information for a DS5 is on your Grant Agreement.

10 How to Complete a Request for Payment Form (DS5) How to SLOW the draw Process 1.Missing Information (site addresses, activity names…) 2.Incorrect Balances 3.Grant Not Encumbered 4.Grant Period Has Ended 5.Funds Exceeds Budget Amount 6.Funds Exceeds the Allowable Limits of 10% or $5,000 7.Small draw

11 They cost us money. They cost you money. You can have $5,000 cash on hand for the life of the grant. This does not violate the 15 day rule. If you submit a draw <$5,000, expect a phone call. If you submit a draw for <$2,000, it may not get processed in a timely manner. Final draws are exempt from those rules. However, if you’re leaving a small balance on your grant consider just drawing it (15 day rule). SMALL DRAWS

12 How to Complete a Request for Payment Form (DS5) Between activity adjustments – Can you do this? –Will the activity budgets remain unchanged? –Is there sufficient balance in the activities? –Will you exceed the $5,000/10% rule?

13 How to Complete a Request for Payment Form (DS5) Between activity adjustments - Situation –You submitted a draw, anticipating expenditures in one activity. –Those came in slightly different than you expected. –You had another project expense in another activity.

14 You didn’t spent it this way, You spent it this way instead. You’ve overdrawn an activity, but it’s by less than the threshold, so you’re okay. You’ll reestablish budget in one activity.

27 FINANCIAL MONITORING VISIT Desk Reviews –Compare reports to budgets and projected outcomes –Approve written request for indirect cost plans

28 FINANCIAL MONITORING VISIT Onsite Reviews –Review financial management and financial files (cash receipts, cash disbursements; –Examine supporting documentation for financial and programmatic activities. –Review internal controls –Write a report with issues and evaluate responses to determine if acceptable

30 OCD Awarded Funds vs. Other Funds –Grantee must be able to report expenditures based on each funding source. –Grantee must have documentation on receipts and expenditures for each funding source. Financial Monitoring Visit

31 Other Financial Requirements –Charges must be based upon Attachment ‘A’ activities. –Indirect Costs can only be charged if grantee has an approved indirect cost plan. –Books must differentiate OHCP awarded funds and all other funds. Financial Monitoring Visit

37 The Draw Report Will Assist With Completing The Status Report. The Report Show All Activities That Has Taken Place During The Grant Period. The Report Will Show The Remaining Budget Balances. Draw Status Report

38 Records  The status of your state audit(s) determines which close letter you will receive once final performance report has been thru the closeout process and is deemed officially closed.  If your state audit(s) have not been closed (we receive this information from our audit department) then you will receive an Administrative Close Letter. This letter states: Once your state audit(s) are closed you will receive a final close letter.  If your audit(s) have been closed, or there was no need for one, then you will receive a Final Close Letter which states: This is a final close letter. The grant is closed, but you have to keep all records for five (5) years from date of letter.
