12 Screen Designs  End User(Members, Non- Members, Instructors, and Employees) Log-in Screen  This screen design will be embedded into the St. Louis Athletic Club web site, which is also to be created for the project. The purpose of this screen is for the end-users to log in to the system.

13 Screen Designs  Registration for an SLAC ID Screen 1:  This screen is used by non- members or first time users of the web based system to register for an SLAC ID and Password. This one time process requires two screens. The first screen, this example, is embedded into the SLAC website and requests personal information from the end user and. The second screen will be described in the next screen shot example.

This one time process requires two screens. The first screen, the previous example, requests personal information from the end user. The second screen, this example, requests the end user to create an ID and Password of their choice, which will be used each time they enter a secured area on the SLAC website.

15 Screen Designs  Member Class Registration Screen:  This screen is the class registrations screen for SLAC members and non- members, but who have registered with the SLAC website.

18 Report Designs  Master List of Instructors:  This report is for the Lessons Department Manager to view a master list of instructors, along with their certifications and availability.

19 Report Designs  Invoices:  The invoice report design is a billing invoice to member and non- members who owe money for SLAC services.

20 Screen Reports  Analysis Report:  Mrs. Coffey wants the ability to analyze the needs and demands of the Lessons Department. This report is a ‘per session’ class enrollment list with waitlist information.
