4 My Friends Discuss why this person/these people are important to you and why you trust them to be your closest friend(s). Ex – These people are important to me because… I trust them to be my closest friends because… A picture of you with your closest friend(s) – (This friend may be a family member)

5 Me In My Community Picture of you in the community (church, sport practice/extracurricular activity/volunteering/su pporting a cause)

6 My Favorite Hobby Something that represents Favorite hobby (listening to music, playing a sport/instrument, learning a craft or skill, spending time in your community/with family)

7 When I Grow Up… What do you aspire to be when you grow up?

8 Making A Difference How would you like to positively impact/make a difference your community? How can you give back to your community, how can you advocate an important value, or what kinds of actions can you take to make a difference in someone else’s life? Ex – I would like to positively impact my community by advocating the importance of total health. This is important to me because I understand that every soul on this planet comes from different walks of life, and the ultimate goal is to achieve happiness, which can solely be achieved through the balance of physical health, mental/emotional health, and social well-being. It is not expected that everyone can successfully perfect this balance, but it does takes soul effort to keep the scale shifting.

9 My Vision My Long-Term Goal – Something you can imagine yourself accomplishing. Write down a Long-Term Goal for yourself. (It can be anything: Ex - fitness, nutrition, grades, a skill, sport, etc…) Make a deadline for this goal (example – By the end of your high school career/freshman year)

11 Climbing Towards Success Action Plan/Stepping Stones Something that’s more within reach that may help you get to your long-term goal. What is it that you would like to accomplish by the end of the day/week/month?

12 Improving My Physical Health What kinds of decisions will you make to stay physically active? Ex - I choose to keep myself physically active because… What are 3 ways that will help you enjoy being physically active? Ex - I have decided to stay physically active by staying committed to my training sessions, taking advantage of the physical demands required by most chores, and make sure that I am fully mentally, and physically prepared to take on these tasks to optimize my performance.

13 Improving My Nutritional Health What kinds of decisions will you make when it comes to your nutrition? Ex – I have decided to become more mindful of my diet on a daily basis because… What are 3 ways that will help you enjoy eating, while still being mindful of your diet?

14 My Support System An object/picture that represents a healthy relationship. List 3 ways that can help you develop/maintain healthy relationships with your family, friends, and other members of your community.

15 Reflection How will these things that are important to you help you live a long/healthy life? Discuss how these things can help you practice abstinence? What is the most important lesson you learned in this class and this year alone?
