1 September 26, 2013 Recovery Connections – Project Update

2 Recovery Connections June proposal to Champlain LHIN: –Develop concrete tools (Health Information Sheet, ER Discharge Planning Checklist, ER Brochure) and support hospitals in completing inpatient discharge planning checklist –Hold Lunch ‘n Learns with hospitals on findings and tools –Develop model for peer and family support in hospitals October proposal to Champlain LHIN: –Call-back service post-inpatient stay –Peer and family support team in hospital settings

3 Call-back service – for any person discharged from inpatient unit who would like a follow-up phone call 1 FTE staff via Distress Centre of Ottawa (access to Mental Health Crisis Line services database) Implemented at Pembroke Regional and Cornwall Community Hospital Updates/in process at Queensway Carleton, Montfort and The Ottawa Hospital

4 Peer Support and Family Support 3.2 FTE paid workers for clients and family members using hospital services at Queensway Carleton and Montfort hospitals Functions provided are similar to mutual support models used in other sectors, including cancer, AIDS/HIV, and diabetes Non-clinical service is complementary to the clinical services provided by hospital staff.

5 What peer support and family support is not…. There is no clinical relationship There is no care coordination relationship The team is there to walk with people, not to help them - can seem counter-intuitive –Team is there to learn about the person, understand and listen –It is a partnership, a relationship, between the team and the individuals Training and orientation process focuses on this distinction

6 Current Activities Implementing communications activities: bi- weekly attendance on-site at hospitals, newsletter bulletins, promotional materials, collaboration with Family Advisory Committee Jobs hired Starting point - 0.4 FTE peer support and family support QCH, 1.2 FTE peer support and family support Montfort Starting point - 0.4 FTE peer support and family support QCH, 1.2 FTE peer support and family support Montfort Montfort Family Support - September Montfort Family Support - September Training and orientation September through November Workers will be phased in gradually beginning in October Workers will be phased in gradually beginning in October Partnership agreement finalized including lead representatives, training, evaluation Review of other programs operations
