19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 1 Update on the implementation of the European Health Interview Survey (EHIS) Eurostat – The European Health Survey System

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 2 The European Health Interview Survey Context: European Health Survey System (EHSS) Annual: EU-SILC (MEHM, some unmet needs) EHIS carried out by the European Statistical System every 5 years: Health Status – EHSM Health Care – EHCM Health determinants (life-styles) – EHDM + Background variables – EBM (demographic and socio-economic characteristics)

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 3 Adoption of the final EHIS questionnaire Final questionnaire for the 1 st round of EHIS adopted at the Eurostat Working Group Public Health Statistics (22 November 2006); available at http://circa.europa.eu/Public/irc/dsis/health/library Click on 'Methodologies and data collections'  'Health Interview Survey'  'European Health Interview Survey (EHIS) 2007-2008 methodology' Soon: methodological guidelines, quality criteria, data submission to ESTAT, etc.

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 4 EHIS – implementation Implementation Vehicle survey(s) to be decided at national level Flexibility – but questions to be respected 1 st round: 2007-2008 (some MS in 2006, 2009) Commission support through grants: Translation and testing of the (new/revised) modules/questions Other preparatory activities (sampling, IT application, staff training, etc.) Financing of 1 st data collection

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 5 EHSM (1): MEHM

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 6 EHSM (2) – Chronic diseases (questions)

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 7 EHSM (2) – Chronic diseases (list)

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 8 EHSM (3) – Work-related health problems

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 9 E4SM – General information European System of Social Statistical Survey Modules aiming at: Rationalising household surveys (existing ones and those under development) Increasing flexibility and timeliness: quickly respond to new Commission needs Annual data collection Different modules, common schema Use existing dedicated national surveys Pilot: 2008; implementation: 2010

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 10 E4SM – structure Annual regular modules: Core variables module (16) Module Information & Communication Technologies (ICT) Modules every two years: EHIS biennial indicators Modules of Adult Education Survey (AES, alternating) Non-annual or irregular modules: Different new modules (e.g. special survey modules developed by SANCO)

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 11 EHIS biennial questions in E4SM Questions should correspond to indicator needs (ECHI and OMC on health care) Questions should correspond to indicators with potential significant evolution in a shorter period than the 5 years frequency of the EHIS survey (i.e. annual or every 2 years) Technical feasibility (seasonality / 4 waves)

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 12 Additional criteria proposed by the CG HIS Topics related to current public health policies Life styles policies (e.g. smoking in public places, at work) Reforms in the health care systems (OMC indicators) Few questions on health status (needs for HC services) Any EU module on health in a population survey shall include the MEHM (3 questions) But: MEHM publication based on EU SILC Same questions as in EHIS (enable comparisons) Questions not to relate to small population groups

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 13 EHIS questions in E4SM related to morbidity Question code Topic HS.1Self perceived health HS.2Longstanding illness HS.3Limitation in activities HS.4Prevalence of 10 relevant main chronic conditions HS.7Accidents resulting in injury + other topics

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 14 EHIS questions in E4SM – chronic conditions

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 15 Diagnosis-specific morbidity statistics Eurostat – Strand on morbidity statistics

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 16 Morbidity statistics – background (1) Comprehensive description of the status of health Health interview survey: self-perceived health Diagnosis-specific morbidity statistics: health as observed by medical professionals Complementary sources

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 17 Morbidity statistics – background (2) Overall aim: establishment of an EU-wide system of consistent diagnosis-specific morbidity statistics Provide a general picture of diagnosis-specific morbidity at population level through Data compilation on a regular basis For a selected set of diseases (shortlist) Within the European Statistical System (ESS) Complement existing public health statistics (COD, health care, HIS)

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 18 Morbidity statistics – background (3) Why? Only limited and scattered data available through the European Statistical System (ESS) Requirement for morbidity statistics from: Community action programme on public health, including ECHI Environment action programme  Need for sustainable data provision for ECHI and other indicator sets

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 19 Activities before 2006 1998: Methods of collecting morbidity statistics – A Eurostat working paper 2003: The London Morbidity Seminar Matrix approach: shortlist – best possible sources 2003: The ad-hoc Task Force Morbidity List 2005: Special meeting of the Partnership on Health Statistics First proposal for a shortlist 2005/06: Pilot project on morbidity statistics Testing of the matrix approach Insurance data

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 20 Morbidity Statistics Development Group (MSDG) (1) New group within Partnership Health in 2006 Bring forward the methodological framework for diagnosis-specific morbidity statistics 5 participants (BE, EE, IT, SE, UK) + Eurostat Group worked June 2006 – spring 2007 Reported to CG HIS

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 21 Morbidity Statistics Development Group (MSDG) (2) Work based on the matrix approach – but further developed Main outcome: recommendations for further pilot data collections Agreed shortlist as framework Principles and guidelines for diagnosis-specific morbidity statistics

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 22 MSDG – Shortlist Diagnosis-specific morbidity – European shortlist (version 6 March 2007) Framework for dissemination of data Based on ICD-10 Independent of sources Selected diseases (criteria based) lack of ambiguity, mutual exclusivity, consideration of ECHI and other existing shortlists, manageable number of diseases, significance, etc Measurements (incidence, prevalence) Short descriptions

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 23 MSDG – Principles and guidelines (1) Principle and guidelines for diagnosis-specific morbidity statistics Fist set of methodological guidelines explaining the approach How to produce best national estimates from appropriate sources for the entries of the shortlist Version 23 April 2007 Enable countries to carry out additional pilot collections of morbidity data

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 24 MSDG – Principles and guidelines (2) Structure and coverage 1.Background and purpose 2.Selected diseases 3.Data requirements 4.Sources 5.How to use sources to meet data requirements 6.Statistical quality (Eurostat quality framework) 7.Submitting data to Eurostat

19 June 2007 Task Force on Major and Chronic Diseases 5 th meeting 25 Pilot data collections Eurostat funding for pilot data collections New MS: multi-country transition facility programme 2005 9 countries participate Start early 2007, 18 months Other MS: Eurostat 2007 grant action Launched May Deadline for applications end June Start late 2007, 18 months
