If revision is necessary, teacher should make necessary revisions and finalize the plan by October 9. Teachers must complete the ‘Components of an Effective Lesson Plan Checklist’ for each observation. Teachers must also complete the ‘Self-Review’ after each observation to be used in the post-observation conference. Every effort will be given to hold a post observation conference within 10 school days of the observation. An agreed upon date will be made between the teacher and administrator if an extension is needed. The Danielson Rubric will be used to rate teacher effectiveness. **Teachers with an I.D.P. will need to meet with the evaluator for a mid-year report no later than January 29. If the teacher fails to submit mid-year evidence, the teacher will be rated as ineffective in these areas on the mid-year progress report. The evaluation of the teacher’s student growth goals and data needs to be submitted by the teacher to the administrator with supporting documentation by May 13. This documentation will be reviewed and factored into the final evaluation report. If no evidence is submitted by the teacher, the teacher will be rated as ineffective in these areas on the final evaluation. 2015-2016 Teacher Evaluation Process Target Dates

14 A formal evaluation may not be necessary for a teachers who received 3 Consecutive Highly Effective Ratings Mid-Year Reviews for teachers on all IDPs Non-Tenured Individual Development Plan vs. Corrective Action Individual Development Plan Corrective Action IDPs focus on Domains which are rated Minimally Effective and Ineffective Special Notes
