3 Objectives of the training Overall objective To provide guidance for the PCT/practice/other care agency teams on how to establish a culture of self care or improve the promotion of self care by patients

4 Aims of the training Aims To explain the policy context of self care within the NHS To develop a local strategy that supports self care To support the development of self care skills To promote a coordinated approach utilising the wide range of skills within the PCT and primary care team To help reduce consultations rates by improving self care behaviours in patients.

5 Summary of the training Timings Three workshop sessions of 2 – 3hr (work in between) Sessions run in sequence over 6 – 12 months Additional meetings possible Audience Members of GP Practice Key members of PCT

6 Basic structure of the training Structure Aims and objectives Using tools and group sessions to develop strategy and enhance skills Developing an action plan (with roles and responsibilities) Reviewing what has been done since the last session
