3 PIA Format The PIA is not to blame or point fingers at anyone. It is presented to analyze the following: – What actions took place? – What actions were beneficial? – What actions could be taken/changed in the future? – Any safety issues and/or equipment failure? – Any suggestions to improve our service?

6 Dispatch Was the alarm reported, received, and dispatched correctly? – Yes. Initially reported/received as a boat fire and then upgraded as the information changed.

7 Size-Up Did the initial arriving officer properly size-up the incident? – Yes, but…….. The officer did report that it was a boat fire with house exposure. One item to ponder for future incidents, is to give a more descriptive picture of what is going on to the incoming units.

8 Was IC established and followed? Yes = E24 established and kept Horizon Command. *Incoming units = “reporting to command”

10 Incident Action Plan Incident Priority = Fire suppression. Primary Goal = Confine the fire to the boat and garage exposure & extinguish fire. Secondary Goals = Salvage & Overhaul

11 Extent of the Fire Boat was fully involved and had extended to the Delta side of the garage & garage attic space.

12 Safety Benchmarks Was 2 in/2 out met? = Yes. Exterior fire initially. ISO Established? = Yes. AC16 Gross decon established? = No! Need to have this established in the future. Was rehab established? = Informal rehab by E24. Did companies report to rehab properly? = No formal rehab set-up. Appropriate PPE in place during fire attack & overhaul? = Yes. Was there air monitoring and appropriate PPE in place after overhaul? = Yes.

13 Rescue Was there a need for rescue efforts and if so were they done appropriately? – No rescue efforts were needed. E24 Officer determined that the occupants of the structure were out of the house during/after 360.

14 Exposures Were there exposures? If so were they addressed? – Yes. The main living area of the house was the main exposure. The next house on the Delta side was also an exposure as there were numerous trees/bushes that separated the two houses. – The exposures' were addressed with a quick knockdown from the exterior of the main body of fire.

15 Overhaul Was the fire completely extinguished? = Yes Was the area of origin preserved as much as possible? = Yes Was the structure secured prior to leaving? = Yes Was the building turned over properly? = Yes

17 Traffic Control Did law enforcement provide traffic control? = – Yes Did you open up the street as soon as possible? = – Yes

18 Utilities Were the utilities handled effectively and within a reasonable timeframe? = – Several comments that the utilities could have been taken care of in a quicker timeframe (i.e. the request, the recognition of where the meters were located).

19 Support Was support 7 requested? = Yes, but cancelled enroute. Was a chaplain offered? = Yes.

20 Communications Were radio communications clear & concise? = For the most part. The initial arriving officer was not on TAC 22 during his 360, so we didn’t hear his 360 report. Also updates on strategy, progress could be more frequent. Were all communications answered & understood? = Yes.

21 Safety Problems/Equipment Failure Were there any safety issues? = Yes – E24 was parked under an electrical line – Electric service meter was damaged and wasn’t noticed initially. – Wrong TAC channel by E24 Officer during 360. – E25 Officer SCBA mask voice enhancer battery was low. – E25 called themselves E24 on communications. Were there any equipment failures? = No.

24 Any suggestions that would improve our service? Any comments? The need to improve status communications during operations. Some crews worked very hard and others stood in a standby position for sometime. Homeowners were on the ground for over an hour without shoes and blankets.
