4. Citizens Gas Indianapolis, Indiana - July 21, 1997 310# 20” Gas Transmission line dropped to 30# for road maintenance 8” parallel line installed by HDD 20” line ruptured when pressure restored 1 killed, 1 injured, 6 homes destroyed, 65 other homes damaged, 75 people in temporary shelters

5. NTSB Findings New 8” pipe within 4” of 20” line Near longitudinal gaping fracture 5.8 ft long Gouge marks from HDD reaming tool along entire length of fracture Plans called for 5 ft separation Pre-HDD field measurements showed 5 to 7 ft could be maintained Did not happen!!
