4 Citizens Gas Indianapolis, Indiana - July 21, 1997 310# 20” Gas Transmission line dropped to 30# for road maintenance 8” parallel line installed by HDD 20” line ruptured when pressure restored 1 killed, 1 injured, 6 homes destroyed, 65 other homes damaged, 75 people in temporary shelters

5 NTSB Findings New 8” pipe within 4” of 20” line Near longitudinal gaping fracture 5.8 ft long Gouge marks from HDD reaming tool along entire length of fracture Plans called for 5 ft separation Pre-HDD field measurements showed 5 to 7 ft could be maintained Did not happen!!

6 NTSB Safety Recommendation 49 CFR 192.614 requires excavation damage prevention programs Excavation damage includes damage from boring

7 NTSB Safety Recommendation NTSB recommended (April 28, 1999) that RSPA (PHMSA’s predecessor), “[w]hen reviewing pipeline operator safety programs, ensure that operators’ damage prevention programs include actions to protect their facilities when directional drilling operations are conducted in proximity to those facilities. (P-99-1)”

8 Advisory Bulletin ADB-99-04 Issued at 64 FR 46967, August 27, 1999 Cited Indianapolis rupture Urged pipeline owners & operators to review O&M and damage prevention programs to consider dangers of HDD near their systems Closed NTSB Safety Recommendation P-99-1

9 Advisory Bulletin ADB-06-01 Issued at 71 FR 2613, January 17, 2006 Cited several excavation-related incidents 11-05 school evacuation in Virginia 6-05 damage to an 800# 30” line (2” nipple) 1-05 damage to a liquid line while getting ready to pig (6” valve knocked off) 11-04 Walnut Creek CA – 5 deaths + injuries EXCAVATION AND BACKFILLING ARE SUBJECT TO OQ REQUIREMENTS!!
