Belle Plaine Activities Fall Eligibility Meeting 2013-2014

To Participate in an Activity at BPHS you need: A Current physical on file at the school updated every 3 years. All MSHSL forms signed and Activities Fees paid Have current insurance Be in good academic standing. No F’s.

Chemical Eligibility No tobacco, alcohol, or other drug use during the calendar year. This includes consumption and possession of illegal substances or drug paraphernalia.

Penalty’s are Accumulative First offense 2 consecutive contests or 2 weeks (14 calendar days) whichever is greater. Attend Insight Class Curriculum Second offense 6 consecutive contests or 3 weeks (21 calendar days) whichever is greater. Third offense 12 consecutive contests or 4 weeks (28 calendar days) whichever is greater. *Category I Policy* - Athletic Activities

Penalty for Category II Activities 1st Violation - Student will miss the next public performance. 2nd Violation - Student will miss the next two public performances. 3rd Violation - Student will miss the next 3 public performances. OR Consequences determined by the Advisor/administration

Academics 1. Student must pass all classes or credits to participate in Activities. No F’s 2. A student failing a class at mid-quarter will be ineligible until the grade is raised to a passing grade.

Academics 1. A student who receives a failing Quarter grade may regain eligibility at mid-quarter of the next grading period if he/she is passing all classes at that time.

4 th quarter F’s A student who receives an F at the end of 4 th quarter will be ineligible for any contests/games that happen before school starts the next year. The student will regain eligibility the first day of school. Participating in tryouts and scrimmages will be allowed.

Attendance Student must be in attendance at school ALL DAY to participate in a practice or contest. Exceptions: Principal’s prior approval, Doctor’s appt., funeral, college visit… If you aren’t sure…call.

Transportation Students are required to travel in school provided transportation unless receiving prior approval from the Principal or Athletic Director, or contacts the coach in person and signs a written statement or permission. Athletes are not allowed to ride home with other students. Athletes may ride home with another parent with written permission given to the AD prior to the event.

MSHSL CODE OF CONDUCT A student whose character or conduct violates the Student Code of Responsibilities or is suspended or expelled is not in good standing and is ineligible for a period of time as determined by the principal.

Face Book/Twitter/Social Media Be careful what information you give out. Be careful what you post. Be careful what pictures you put on those pages. If information / pictures are posted on these pages and it shows a rules violation and is brought to our attention, we have to investigate. Code of Conduct violation may occur

ONLINE PAYMENT Log into “Infinite Campus” Click on the “Payment” icon Fill in the amount you need to pay after the activity you will be involved in. Click on the account type and provide the information necessary. You will be able to pay Online after August 12 th.

What do I need to turn in? MSHSL Athletic Eligibility Statement (both sides) Insurance Waiver (both sides) Medical Consent form Pay the athletic fee (unless you will pay Online)

Coaches Meetings Fall Play - Mrs. Jacobson’s Room Tennis – Band Room Volleyball - North Gym Football – South Gym Cross Country – Choir Room Visual Arts – Art Room Please make sure you sign up for your Activity in the meeting.
