3 Wisconsin Stroke Coalition Wisconsin Stroke Coalition (WSC) is a group of organizations dedicated to the development and implementation of stroke systems of care in Wisconsin, along with the advancement of key stroke initiatives to reduce the burden of stroke, improve patient outcomes and support a healthy population relative to stroke prevention and risk factors. 5 Current focus: recruitment of members, committee chairs Begin committee work Re-create the WI Stroke Plan in alignment with current recommendations for stroke care Alternate between quarterly in-person/call meetings for the steering committee. 8 Proposed Regions 7 regions: 1.Greater Milwaukee/Southeast 2.Madison/South Central/West 3.Green Bay/Northeast 4.Fox Valley 5.North Central (Marshfield/Wausau) 6.La Crosse (West) 7.Eau Claire (Northwest)

9 Regional Plan Timeline Milwaukee Regional Conference is scheduled for June 14, 2013 at the Italian Community Center Madison Regional Conference will occur in fall 2013 Plans for 2 additional regional conferences to occur in 2014

10 Benefits Through collaboration with EMS and participating hospitals: Develop a Regional Stroke System of Care (SSOC) –Hospital Designation: Non-Acute Stroke Capable, Acute Stroke Ready, Primary Stroke Centers, Comprehensive Stroke Centers Enhance identifying stroke in the field by EMS Improve field pre-notifications from EMS to ED Ensure that patients go to hospitals that will treat stroke patients.

11 No Mandating of Care There is no mandating where patients go, but rather ensuring there is a regional system in place Regardless of the hospital the patient goes to, there is a regional protocol to get them to the the site that has the capabilities to treat their stroke health care needs

12 Regional Conferences Goals Development of a regional stroke system of care –Regional leadership team (also members of WSC) Development of regional EMS protocols –EMS standards with standard protocols –EMS education when needed Establishment of referring hospital transfer protocols –Engage our Critical Access Rural hospital in stroke education and quality care Development of regional report card to have data feedback between providers and ensure data-driven practices
