2 The Chronic Disease Prevention Initiative (CDPI) Designed to address the common modifiable risk factors : ▪Support people to live smoke free ▪Encourage healthy eating ▪Promote active living …through local actions, evidence informed approaches and the creation of supportive environments for healthy lifestyle choices and behaviours.

5 Community Leading the Journey Local Planning committees and champions Community engages volunteers Community health information collection and effective use in building supports for healthy living Action plans with per capita funding Starting where community is at and address barriers to participation Find and match community needs with resources Community pride & “yes-we-can” attitude

6 Community Leading The Journey 83 communities in all participating regional health authorities including 21 First Nations and 7 Métis communities For example: Train the Trainers – growing community expertise Making it easy for people to connect e.g. “Where to get active” directory Power of community success

7 The journey – RHA coordinated Built on existing processes and structures for regional planning and coordination Worked with communities at risk for Chronic Disease Demo project design - RHA board buy-in Redefine roles, relationships & partnerships Draw on evidence for decisions and planning Make it easy for communities to participate

8 The journey – RHA coordinated Build commitment and supports that enable upstream prevention Redefine roles; engaged other providers Develop relationships and partnerships For Example: Developed Regional Committees (training plans, surveillance) Shared resources – Interlake displays, communication tools

9 The journey – government supported Funding through Public Health Agency of Canada and Manitoba Health and Healthy Living Leadership, Accountability, Evaluation Support of committees for communication and coordination

10 The journey – government supported For example: Health in Common – website to support CDPI needs (tools, processes, resources, list-serve) Linkage to provincial partners such as Northern Healthy Foods Initiative New funding for healthy living and chronic disease prevention teams for regions to enhance existing programs
