5 Integrated Children’s Centres Integrated, citywide service led by health visitors who are seconded into the Council and based in CCs HVs register parents at the new birth visit HVs assess all children under Healthy Child Programme – increasing ante natal, 1 and 2 year reviews Use the health visiting levels of service: –community –universal –universal plus –universal partnership plus (includes CAF, CP, LAC) HVs directly supervise Council CC staff Shared data and aligned reporting procedures but two IT systems

6 Integrated CCs continued HVs act as the lead professionals for CAFs for children under 5 Directory of evidence based interventions Health Visitors trained in, deliver and supervise Council staff to deliver parenting programmes Training passports for health and council staff Family Nurse Partnership based in and managed as part of the CC service Early Parenting Assessment Programme – for mothers at risk of losing babies based in a CC
