1 SEMS allows you to create reports based on your files. This guide will give you the steps to create a report on the SEMS site using the Question Wizards function. You will also learn how to save, modify and export reports. More information on reports can be found in the online Help section of SEMS. Step 1 Click on the Report Center icon on the left side of the screen Any report you have previously worked on will be listed here as well as icons for several suggested reports. 2 SEMS allows you to create reports based on your files. This guide will give you the steps to create a report on the SEMS site using the Question Wizards function. You will also learn how to save, modify and export reports. More information on reports can be found in the online Help section of SEMS. Click on the Report Center icon on the left side of the screen Any report you have previously worked on will be listed here as well as icons for several suggested reports. On the right side of the screen in the Create Report section select Question Wizards. 2 Safety Event Management System (SEMS) Creating Reports Step 1 2

3 Step 3 Select the Risk Module radio button to make a list of volumes appear. Step 4 Select Incident Volumes. The page displayed below will come up. Creating a Report With the Question Wizards Step 5 Click the group field link and select General Event Type. This will aggregate the numbers of incidents by event type. Step 6 Click the file state and choose Multiple Selection. Check the boxes for New, In-Progress, and Closed. Step 7 Click the date field link and select Entered Date under the Incident section. This will allow you to filter out by the date the file was entered. Note: Be sure you select the Entered Date from the Incident section. If you select the Entered Date from the Follow-ups, Incident File, or Tasks sections… the chart will not run correctly. 3 Step 8 Click the date range link, select This calendar year, and click OK. This will filter the report to show only events entered so far this year.
