3 Long Term Conditions – Key Facts  In Scotland an estimated 2 million people live with one or more condition.  Prevalence is set to rise significantly as the Scottish population ages.  Long term conditions are the leading cause of death world wide and account for 85 per cent of deaths in the UK.  Long term conditions account for around 80% of GP consultations, 60% of in-patient bed days. 70% of emergency admissions and 80% of prescribed medicines.  There is strong two-way correlation between physical long term conditions and mental health problems.
