2 Avianca Flight 052 -Thursday January 25, 1990 -Colombia → New York -Laureano Caviedes/Mauricio Klotz -203 flights delayed for inclement weather -Held by ATC three times on way

3 Avianca Flight 052 (Cont.) -Delays in air: - 19 minutes - Norfolk, Virginia - 29 minutes - Atlantic City - 29 minutes - Kennedy Airport -Final descent - Wind shear - Auto-pilot malfunctioning - Go around → engines blown → crashed -73/158 dead

4 Korea Flight 801 -August 5th, 1997 -From Kimpo International Airport in South Korea to Guam -Experience of Flight Captain - 8,900 hours of flight time - 3,200 hours in jumbo jets - Had flown to Guam eight times before - Flight safety award - In excellent health

5 Korea Flight 801 -Took off at about 10:30 p.m -Began its descent at 1:30 a.m -Raining; couldn’t see the runway -Suggested a “missed approach” -“Go around”...”GO AROUND” -At 1:42 a.m, crashed into Nimitz Hill -228/256 (89%) dead

8 Air Florida Flight 90 -1982 just outside Washington D.C. -Inadequate communication between two pilots -False Security "Lets check those wing tops again since we've been sitting here a while"

9 Air Florida Flight 90 - Solution: If the co pilot/ officer just told the pilot to defrost the wings instead the crash would have never happened, the sense of mis- communication occurs because the co pilot felt as though he knew better - Rushed the situation --> failed to react properly to ice --> downfall

10 Important Ideas - Mitigated Speech – any attempt to downplay or sugar coat the meaning of what is being said - Was the difference in the Air Florida crash (page 197) - Crew Resource Management (197) - Ratwatte “if you see someone doing something stupid speak up" (page 198)

11 Important Ideas - Power Distance Index – concerned with attitudes toward hierarchy (authority) (204) - Low PDI  plane is flown by two equals - High PDI  captain is leader - Korean Flight: pilot makes error --> co-pilot doesn't try to correct him - Once Korea found cultural problem --> fixed recent struggle of losing planes

12 Why This Chapter Was Included -Shows that without proper communication you will crash and burn -Shows that one of the secrets for success is communication
