2 Agency Goals Related to Transition: Improve School-to-Work Transition Services and outcomes Improve collaboration between all agencies involved in transition activities Develop IPE for all eligible students prior to exiting high school

3 Transition Student: A secondary school student with a disability pursuant to IDEA and/or the Rehabilitation Act, who is an applicant or eligible individual for Vocational Rehabilitation services and becomes a signed applicant for services before exiting a secondary school program.

4 VR Counselor Assignments Develop and maintain effective working relationship with assigned high school Provide consultative services Ensure timeliness in processing referrals, determining eligibility and working with student to develop IPE

5 DVR TIMELINES: Referral – September through October Eligibility – By December Comprehensive Assessment/Career Counseling/Guidance and Counseling - January through March/April IPE Development – March through May

6 Eligibility: Individual with a physical or mental impairment; which results in a substantial impediment to employment; who can benefit in terms of an employment outcome from vocational rehabilitation services; and VR services are required in order for the individual to prepare for, enter, engage in, retain or regain employment

7 Services: Guidance and Counseling/Career Counseling On the Job Training Support in Post-Secondary Education Apprenticeships Assistive Technology Job Seeking Skills (soft skills) Job Placement and support Job Coaching
