3 Who is LabOne? l A national laboratory headquartered in Lenexa, KS n Started in 1972 n Three lines of business-Risk Assessment, Healthcare & the Employer Services Group. n Over 300,000 tests performed daily (over 90 million tests annually) n Fully Accredited and Certified

4 What is the Lab Card ® Program? l Lab Card is an additional benefit offered to you by your employer, Drury University. l Lab Card does not replace your existing benefits, it provides you with an option to receive laboratory testing at no cost to you - 100% coverage. l Lab Card is a patient-driven program - you decide where your doctor sends your specimens and your employer pays 100% of the charges IF your specimens go to LabOne.

5 What does the Lab Card cover? l Lab work covered by the Lab Card: n Blood testing (cholesterol, CBC, Lipid Panels) n Urine testing (urinalysis) n Pathology (PAP smears, mole biopsy, skin biopsy) n Cultures (throat cultures)

6 What is NOT covered by the Lab Card? l Lab work while hospitalized l Lab work while in the emergency room l Lab work needed on an emergency (STAT) basis l Testing not covered under your current medical plan l Non-laboratory work such as mammography, x-ray, imaging or dental work l Specimens not processed by LabOne

7 3 2 1 The Member initiates the process by showing their Healthcare Card at their physician’s office and again when the physician states that tests are going to be ordered. It is important to let the office know that the specimens need to be sent to LabOne. The physician can collect the specimens and calls LabOne for pickup OR the member can choose to go to a contracted LabOne collection site where there will be NO charge for the collection. LabOne performs the tests and faxes/sends results to the physician, generally the next day. How the Lab Card Works

8 What do I have to do differently at my physician’s office? l Show your Healthcare card when you check in and let the office personnel know you intend to use your Lab Card for your lab work. l Your doctor’s office will need to understand that your lab work has to be sent to LabOne in order for you to receive 100% coverage. l To insure that you will not be charged for a collection you can go to a LabOne contracted collection site.

9 What if my physician is unfamiliar with LabOne? If your physician is unfamiliar with LabOne, there are 3 ways to educate your physician. Ê Call LabOne’s 800 number â 800-646-7788 â A LabOne representative will contact your physician to educate him or her on the Lab Card Program. Ë Provide your physician information via our website â www.LabCard.com Ì Have your physician call LabOne’s 800 number, and a Customer Service Representative will answer any questions that he or she might have

10 What if my physician does not collect specimens in his or her office, does not want to send to LabOne or will charge me for the collection? l You can choose to use a LabOne contracted collection site to collect your blood and urine specimens at no charge to you. â Request a prescription from your physician for the tests they would like to have performed â Contact LabOne’s Clinical Client Services @ 800-646-7788 or go online to www.LabCard.com to find the site closest to you â Insurance Physicals of SW MO-1736 E Sunshine, # 300  RN-Med Ex, Inc- 2837 S. Fremont â Take your prescription to the LabOne collection site and provide the prescription and your Healthcare card to the site. Be sure to remind them that you have the Lab Card â All results will go back to your physician, typically within 24 hours

13 Review l When your outpatient lab testing is done at LabOne, your employer pays 100% of the lab testing charges - there is NO cost to you. l This is an employee-driven program - you must identify yourself as a Lab Card member. l Always remember the 800 Customer Service number on the back of your ID card, if you have any questions on the Lab Card Program. l Please be sure to visit our website, www.LabCard.com, for more information, including a place to request physician education and a current list of our contracted collection sites.
