OB Authorization & Initial Assessment Form WHO All practitioners who seek obstetrical care authorization for patients enrolled in DC Medicaid or Alliance MCO plans; Completion of the OB Authorization & Initial Assessment form, (pages 1 & 2) is MANDATORY; and The form does not need to be filled out by a physician. Note: A new form does not have to be completed each time there is new information.
