3 Hire above Minimum Form Applicant’s name Position title and class code to include internal title (if applicable) Pay band and salary range Applicant’s current salary Proposed salary Comparable salary information (department, agency, statewide) Justification statement(s) to include not only that the applicant exceeds the minimum requirements but also a description of why the salary is needed to hire the individual (i.e., market, difficulty in recruiting/retaining, most qualified and little time needed to train candidate) Hire date (if known) Authorized approval and date

4 Classification  Minimum Training and Experience Requirements  Ensure classification actions are approved prior to the effective date of the action  FLSA Designations  Essential/Marginal Functions

5 Temporary Salary Adjustments  Ensure that TSA that extend beyond 12 months are approved by SHRD  Duration of temporary assignment
