9. Surrogate Parent Assignment Process A Surrogate Parent is required when no parent can be identified, when the whereabouts of a parent cannot be determined or when a student is a ward of the state. Determination of Need for a Surrogate  At Student Enrollment Principal Requires Parent Information  If No Parent Identified then Principal Requests Assignment of a Surrogate OR  Student in the Custody of Department of Social Services (DSS) so Parent Information Reviewed Parental Rights Terminated or Planned to Be Terminated Foster Parent, Relative or Custodial Relation Meets Definition of Parent  If No Parent then DSS Requests Assignment of a Surrogate Parent

Surrogate Parent Training Components:  Surrogate Parent Certification Program Participation in Yearly Basic Training  Surrogate Parent Code of Ethics Including Confidentiality Statement  Certified Surrogate Parent Information & Conflict of Interest Agreement LEA Contacts Appropriately Trained Surrogate Parent  Assignment on a rotating basis and based on factors including special skills of surrogate, where surrogate resides or works and demographic congruence  If contact accepts the assignment then additional information will be shared.
