5. 5 PIN Letter Neal – draft sample letter to Reviewers (to be faxed or mailed) to provide them with their PIN (person_id) for registration. In order for you to log on to the Internet Assisted Review Module for the first time and establish your personal password, you will need a Personal Identification Number (PIN). This is to be used only on the first occasion that you access the system and create your own personal USER NAME and PASSWORD. YOUR PIN IS AS FOLLOWS:_____________________ Please keep your PIN in a safe place until you have initially logged on to the system and established your personal USER NAME and PASSWORD. For subsequent system access you will use these latter user name and password, so generate ones that are easily remember by you. In the event you forget your USER NAME and PASSWORD, please call your SRA.
