3 What to do FIRST! (Continued) 3 Complete the FFATA form. An electronic version will be sent to you. Obtain your CCR (Central Contractor Registration) at www.irs.gov www.irs.gov Complete General Statement of Assurances (form link located on ADE home page/A-Z link/Grants Management). Sign in blue pen. Make sure all signatures are correct and signed by the appropriate person. * Note: GSA and CCR are required annually.

4 How to Submit Required Information 4 Send Entity Profile Form electronically. It will automatically default to Sheri Partridge. W-9, GSA and FFATA submission options: Hand deliver original documents to Sheri with original signatures in blue. NO COPIES! Snail mail original documents to Sheri with original signatures in blue. 5 Correct Contact Information 5 Your contact information, especial mailing address, must be correct and up to date. Negative results of outdated or incorrect contact information: Return to Sender Cancellation of payment Reissuing of payment and its subsequent delays Change of address: Common logon

6 AZ CSP Time Line The new AZ CSP Project Year begins April 1 of each calendar year. Your AZ CSP Project Year ends on March 31 of each calendar year. 6

7 AZ CSP Time Line Now that your application has been approved, you will submit your April 2013 funding request through the Special Request process. Email your special request for funds to Sheri.Partridge@azed.gov. Sheri.Partridge@azed.gov It will take a minimum of two weeks to process your request. 7

8 How to fill out the Special Request 8 Create ADE Budget Form format including Line item Cost Description You may submit in either of the following program formats: Excel MS Word Description information: Actual items or services Do not include Rationale or Alignment

9 AZ CSP Time Line To make sure your May and June payments are distributed in a timely manner, submit Special Request for payment to Sheri by the 18 th of the previous month. July payment is submitted through GME Payment Form. Access GME through ADE Home Page/A-Z Link/Grant Management. 9

10 AZ CSP Amendments You must submit an amendment whenever there is a fiscal or programmatic change to an approved grant/project over 10% of the line item. This includes requests that alter line item allocations, budgetary changes or description change of a line item. Amendments can be submitted no later than 90 days prior to the project end date, December 30, 2013. 10

11 AZ CSP Completion Report A Completion Report (CR) is a report of actual expenditures for all grants/projects. Submission is required for all grants/projects. Due within 90 days after grant/project end date. 11

12 Monitoring 12 Monitoring Site Visits AZ CSP staff conducts onsite visits multiple times each project year to monitor charter schools receiving AZ CSP grants. The purpose of each site visit is to determine how well the sub-grantee is meeting requirements and guidelines of the grant (SEA Monitoring Indicator 2.5 Subgrantee Monitoring: The SEA monitors subgrantee projects to assure approved grant and subgrant objectives are being achieved.). Visits will focus on the following areas: 1. Governance 2. Academic Program 3. Business/Finance
