1 Know the Codes Code 22Code 22 - indicates a pediatric arrest. Is activated by dialing “7444” (or pushing the code button on the wall). The caller will tell the operator the location of the code. Example, Pediatric department room 431A. Code 44Code 44 – indicates an adult cardiac arrest. Is activated by dialing “7444” and informing the operator of the location of the code or activating the code button on certain depts. Code 55Code 55 – indicates that a fire or suspected fire has been detected. Is activated by pulling the fire alarm closest to the location. Then, if possible, when the site is cleared, call the operator at ext. “6500” with additional information pertaining to the fire. Responding people will be security, plant operations, house supervisor, and one employee from each department with a fire extinguisher. Code 88Code 88 – Multiple trauma; internal disaster (bomb threat, explosion). All nursing personnel will follow instructions identified by department manager/charge nurse for both code 88 and 99. Code 99Code 99 – indicates that an external disaster has been declared. This will be announced as level 1, 2, or 3. The level is determined by the number of expected casualties/patients.

2 Know the Codes Code OrangeCode Orange – indicates that a potential life- threatening event has or may occur. Example would be some one with a gun, knife or explosive device. A Hazardous Material Spill or disruptive behavior by visitors. It is activated by dialing “6500” and notifying the operator of the location and type of occurrence. This will be handled by security and no one other than security will respond to the location announced. All other employees are to stay in their work area until an all clear announcement is made. Code SilverCode Silver - this code is activated when there is a person or persons on the premises that has used or continues to use a gun against person(s) on the Geisinger property. When you hear the overhead announcement or gunshots, apply the Mindset of Survival Skills for everyone in your area. Evacuate (go to safest place possible), Hide Out (hide in an area outside shooters view, block entry, turn off lights and turn off anything that can make noise) Take Action (as a last resort if your life is threatened) To activate the Code Silver dial “6500” and tell the operator that you would like to activate the code silver and tell them the location (building/floor/dept). The operator will notify security and the local police of the incident and announce it overhead 3 times.

3 Know the Codes Code BCode B – indicates that a child or infant is missing or unaccounted for from the OB or pediatric dept. The employee that is made aware of the situation will dial “6500” and notify the operator that you are activating a Code B alert and inform him/her of the sex and age of the child as well as the department from which it is missing. An example of the announcement would be, “ Code B, F-3, OB Dept. which would translate to a three year old female missing from the OB Dept. If an infant is abducted from the nursery the operator will announce, “Code B Medical Center OB/GYN. Both announcements will be made three times. All employees not involved in a life-saving issue are to go to the nearest window, door or exit and observe then report any unusual occurrences to security.

4 Code AdamCode Adam - indicates a child or infant is reported missing from any dept other than OB/Peds. To activate, the employee made aware of the situation will dial “6500” and inform the operator that they would like to activate a “code adam” alert. A detailed description of the child is given along with the child’s name, age, and location last seen. The operator will then announce overhead “Code Adam, Thomas, age 2, ED”. This would indicate a 2 yr old named Thomas was reported missing from the ED. Immediate response actions by all other staff would be: Depts located near exits should monitor the exit path and door Ask patients/visitors with children who resemble the des cription to wait until management arrives, then ask the child his/her name and whether the adult is his/her parent Escort the parent of guardian of the missing child to the front of the facility where they entered to assist in identifying the child; After hearing the overhead announcement all available staff should begin looking for the child. Check all restrooms etc on your dept Report any suspicious activity to security.

5 Know the Codes RRT – Rapid Response Team. The RRT consists of nursing supervisor, respiratory therapists and hospitalists. The RRT is activated by dialing “6500” and informing the operator that you are requesting the RRT and the location. Activate the RRT for all chest pain patients or any patient whose condition has taken a turn for the worse. This is not to take the place of notifying the patient’s physician but is done simultaneously.RRT – Rapid Response Team. The RRT consists of nursing supervisor, respiratory therapists and hospitalists. The RRT is activated by dialing “6500” and informing the operator that you are requesting the RRT and the location. Activate the RRT for all chest pain patients or any patient whose condition has taken a turn for the worse. This is not to take the place of notifying the patient’s physician but is done simultaneously. Remember no RRT is a wrong RRTRemember no RRT is a wrong RRT
