5 Case Management Identify those traits that are needed by a professional nurse case manager: Communication, independence, flexibility, creativity. Case Managers are typically NOT direct care providers, i.e., they do not perform advanced clinical nursing skills or techniques on clients. Identify patients / clients in need of nursing case management services Typically, these patients are in need of a case manager to coordinate care across a broad spectrum of services, e.g., skilled nursing care, placement in facilities, durable medical equipment, pharmaceutical services, physical & occupational rehabilitation. Typically not acute cases, or those not requiring extensive follow-up. Note that nurses serving other functions or having other job titles, e.g., public health nurse or school nurse or occupational health nurse may also be performing “case management” and be working as a “case manager” if they are caring for individuals with complex health problems.
