3 Occupational Therapy Immediate Intervention  Assess Thomas’s needs- home environment, school environment making necessary interventions  Liaise with Thomas, parents and Social Services so an informed choice can be made regarding Thomas’s education

4 Occupational Therapy Mid-Term Intervention  Liaise with Social Services, Education Authority and Disability Officer to make necessary changes to enable Thomas to access new school  On going assessment of Thomas’s needs as he develops (work, rest and leisure)  Work with family to help set up coping strategies enabling them to adapt to a changing environment (house, school, support network)

5 Occupational Therapy Long-Term Intervention  On going assessment of Thomas’s needs i.e. Junior School to Secondary School  Provide continuing support for family i.e. information about Voluntary agencies/services  Act as advocate for family by liaising with Social Services, Education committees and Disability Services to identify areas of need/entitlement
