7 Log-in using your UA credentials (normal username and password)

8 Next…..choose the appropriate incident you wish to report Note: Anyone may complete an incident report on behalf of another injured employee 0r non-employee (to include students) Almost there! Next, click on “Submit a new incident” to see the incident report options (below)

9 Congratulations, you may now complete the report you selected!
